Objective To explore feasibility of building health management for the elderly under community service mode of "integrated medical and nursing care". Methods Choose 120 lonely elderly people with chronic diseases such as hypertension and diabetes in our community from June 2016 to June 2018. Divide them into two groups, 60 cases in control group were treated with routine community management, and 60 cases in observation group with elderly health management under service mode of " integrated medical and nursing care". Results Physical health risk and mental health status scores of observation group were lower than control group, and daily living ability and social support scores were higher than control group, with obvious difference, (P＜ 0.05). Conclusion Elderly health management under community service mode of "integrated medical and nursing care" combining with routine community management services can improve health management level and life quality of the elderly.%目的 探讨构建社区"医养结合"服务模式下老年人健康管理的可行性.方法 选取2016年6月-2018年6月本社区老年人120例,所有老年人均独居并患有高血压、糖尿病等慢性疾病,将老年人分为两组.对照组60例,给予常规社区管理;观察组60例,在"医养结合"服务模式下开展老年人健康管理工作.结果 观察组老年人的躯体健康风险、精神健康状况评分均低于对照组,日常生活能力、社会支持情况评分高于对照组,差异明显(P＜0.05).结论 在社区"医养结合"服务模式下进行老年人健康管理,结合常规社区管理服务,提高老年人健康管理水平,改善老年人生活质量.
