结扎速血管闭合系统闭合胆囊颈管和血管在LC中的应用

摘要

目的 探讨腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)时应用结扎速血管闭合系统(LigaSure vessel sealing system,LigaSure)闭合胆囊颈管和行胆囊动脉止血的临床可行性,比较其与夹闭法、双极电凝法间存在的差异.方法 2010年12月～2011年5月,随机对50例LC患者术中应用LigaSure闭合胆囊颈管和行胆囊动脉止血,将其临床效果、手术耗时,术后住院日和并发症发生情况与同期50例夹闭法、50例双极电凝法的患者进行统计分析.结果 50例应用LigaSure患者手术均获成功.手术耗时、术后住院日和并发症发生情况与同期夹闭法、双极电凝法患者差异无统计学意义(P＞0.05).结论 LigaSure用于LC中闭合胆囊颈管和行胆囊动脉止血是安全、可行的,与双极电凝法相比,LigaSure更易掌控,安全、可靠系数更高,在克服夹闭法不足方面,LigaSure与双极电凝有相同效应.在同时具有双极电凝和LigaSure两种设备的情况下,建议首选LigaSure.%Objective To study the clinical feasibility of LigaSure vessel sealing system ( LigaSure) for closure of the cystic duct and artery in Laparoscopic Cholecystectomy ( LC) , used to compare with the clipping method or the bipolar coagulation method. Methods 50 cases underwent LC with the cystic duct and artery sealed with LigaSure only from Dec 2010 to May 2011, comparing with the clipping method or the bipolar coagulation method in clinical effect, operation time, postoperative hospital stay and complications. Results 50 operations were successful, no difference with the clipping method or the bipolar coagulation method in the same time about operation time or postoperative recovery ( P ＞ 0. 05 ) . Conclusion Using LigaSure to seal the cystic duct and artery is safe and feasible, it can overcome the shortcomings of clipping method like Laparoscopic bipolar coagulation,but it better in control and security . If you have Laparoscopic bipolar coagulation and LigaSure two devices, LigaSure is recommended preferred.
