Objective To investigate the short term clinical effect of tolvaptan on elderly patients with refractory ascites and hyponatremia due to liver cirrhosis. Methods 41 patients were divided into treatment group(21 cases) and control group(20 cases) . The former had taken tolvaptan orally besides conventional treatment for 5 days,meanwhile,the latter had taken placebo in-stead. The clinical effect and side effect were observed. Results After 14 day treatment, in treatment group, 5 cases were markedly effective,14 cases effective,2 cases ineffective,and the effective rate was 90. 48%. When comes to control group,the da-ta were 2,9,9,respectively,and the effective rate was 55%. The difference between two groups was significant(P<0. 05). As re-gards the side effects,only 3 patients felt thirsty,and 2 patients felt dry. Conclusion Taking tolvaptan orally for a short term was effective and had no severe side effect for elderly patients with refractory ascites and hyponatremia due to liver cirrhosis.%目的 观察托伐普坦片治疗老年肝硬化难治性腹水伴低钠血症患者的疗效及安全性. 方法 41例老年肝硬化难治性腹水伴低钠血症患者随机分为治疗组(21 例)与对照组(20例). 治疗组在常规治疗基础上加用托伐普坦片口服,而对照组加服安慰剂,共治疗14d. 观察两组治疗后临床效果及不良反应. 结果 治疗14d后,治疗组显效5例,有效14例,无效2例,有效率90. 48%;对照组显效2例,有效9例,无效9 例,总有效率为55. 00%,两组疗效比较差异有统计学意义(P<0. 05). 在本次试验中治疗组出现口渴3 例,口干2 例,未见其它不良反应. 结论 在老年难治性肝硬化腹水伴低钠血症患者中,短期口服托伐普坦,疗效显著,能进一步改善症状和体征,且无严重不良反应.
