艾滋病合并肺结核与单纯性肺结核的CT表现对比研究

摘要

目的：探讨艾滋病( AIDS)合并肺结核与单纯性肺结核的CT表现差异。方法回顾性分析142例AIDS合并肺结核患者的CT表现,并与127例单纯肺结核患者的CT图像进行对照。扫描结果根据发病部位、范围及病变性质进行对比分析。结果病变部位及范围：病变累及双肺、两叶和两叶以上、上叶尖后段及下叶背段,两组间比较差异无统计学意义(P>0．05)；病变累及胸膜或淋巴结,两组出现率比较差异有统计学意义(P<0．05)。病变性质：斑片状或大片状实变阴影、大片实变合并多发空洞、纵膈内淋巴结肿大、胸腔积液及心包积液两组间比较差异有统计学意义(P<0．05),而单纯空洞、多发结节影在两组间的出现率差异无统计学意义(P>0．05)。结论 CT能显示AIDS合并肺结核的各种影像特征,AIDS合并肺结核的病变部位不典型,累及范围广泛,大片状实变、纵膈内淋巴结肿大、胸腔积液及心包积液的出现率较单纯肺结核高。%Objective To explore the CT imaging features difference between pulmonary tuberculosis with AIDS and pure pulmonary tuberculosis. Methods The CT features of 142 pulmonary tuberculosis patients with AIDS were analyzed retrospective-ly according to the location and character of scan findings and in comparison with that in 127 patients with pure pulmonary tubercu-losis. Results The location and range of lesions:distributed in two lungs,more than two lobes,apicoposterior segment of lobus su-perior and superior segment of lower lobe had no statistical significance between two groups(P>0. 05). The occurrence rate of le-sions involving the pleura or lymph nodes between the two groups was statistically significant(P<0. 05). Features:patchy or large sheet consolidation shadows,large consolidation combined with multiple cavities,swollen lymph nodes in the mediastinum,pleural effusion,and pericardial effusion,the difference between the two groups was statistically significant(P<0. 05),however,ocurrence rate of the mere cavities,multiple nodules between the two groups was not statistically significant(P>0. 05). Conclusion Ima-ging features of pulmonary tuberculosis with AIDS can be showed by CT,the lesions of pulmonary tuberculosis with AIDS are atypi-cal,Widely-ranging,large sheet consolidation, swollen lymph nodes in the mediastinum, pleural effusion, and pericardial effusion rate of occurrence are higher performance compared to pure pulmonary tuberculosis.
