老年急性缺血性脑卒中患者下丘脑腺垂体激素水平与预后相关性分析

摘要

Objective To investigate the prognosis and correlation analysis between hormonal levels of hy-pothalamus-pituitary and acute senile stroke patient .Methods One hundred stroke patients who aged sixty-five and above were collected in this experiment .The hormonal level of cortisol ,TT3 ,TSH and sex hormone were tested by radioimmunoassay .Diagnosis and classification of stroke were analyzed according to MRI +DWI .The severity of the stroke and prognosis were analyzed according to NIHSS and mRS.Results The prognosis of stroke patients was worse due to high levels of cortisol .But it was better for those with high levels of male hormone ,and female hormone .Conclusion There is a significant correlation between hormonal levels of hypothalamus-pituitary and acute senile stroke patient.%目的：探讨老年急性缺血性脑卒中患者下丘脑腺垂体激素水平对预后的影响及相关性。方法收集100例符合诊断标准的急性脑卒中老年患者（≥65岁），用放射免疫法测定皮质醇、促甲状腺激素（TSH）、性激素水平。脑卒中的诊断及分级采用磁共振成像（MRI）＋弥散加权成像（DWI）的结果，脑卒中严重程度与预后用美国国立卫生研究院卒中量表（NIHSS）评分和修订致残量表（mRS）评分。结果皮质醇水平显著增高的患者，其预后越差（P＜0．05），雄激素水平越高的患者其预后越好（P＜0．05），雌激素水平越高的患者其预后越好（P＜0．05），TSH水平越高的患者其预后越差（P＜0．05）。结论下丘脑腺垂体激素水平与老年急性缺血性脑卒中患者预后有显著相关性。
