毛囊皮脂腺囊性错构瘤间叶组织免疫病理分析

     

摘要

Objective To measure the immunohistochemical expression of CD10,CD31,CD34,EMA, P63,S100,ER and PR in folliculosebaceous cystic hamartoma(FSCH)and its clinical significance.Methods Im-munohistochemical markers above were detected in 5 FSCH cases.Their expressions were measured by Ventana automatic immunohistochemistry stainer.The expression and distributed patterns were observed.Results CD10 had been consistently noted in the nuclear and cytoplasmic membranes of mature sebocytes.CD31 was positive for blood vessels and CD34-positive spindle cell in the surrounding stroma of lesions.EMA is positive for sebaceous gland,eccrine gland and negative for keratocytes.Nuclear p63 staining was strong in the basal,suprabasal cells of the epithelial units and sebaceous cells.No difference in S100 was found between ectopical and subcutaneous tis-sue.Staining for ER and PR was negative.Conclusion Different stroma elements may affect the differentiation of cystic wall.FSCH is a late-stage in trichofolliculoma differentiation.%目的 观察CD10、CD31、CD34、上皮膜抗原(EMA)、P63、S100、雌激素受体(ER)、孕激素受体(PR)在毛囊皮脂腺囊性错构瘤(FSCH)的表达,并探讨其临床意义.方法 以石蜡包埋的5例FSCH皮损组织标本作为研究对象,使用全自动免疫组织化学染色仪对5例FSCH进行上述标记物检测.结果 CD10在皮脂腺中胞核和胞膜阳性表达;CD31主要在间质内血管,而CD34阳性表达主要位于毛囊皮脂腺单位中周围基质梭形细胞等纤维成分;EMA在皮脂腺、汗腺呈阳性,角质形成细胞染成阴性;P63核染色强阳性位于表皮、漏斗部基底层及以上表皮,在皮脂腺中呈胞核阳性表达;S100阳性染色在异位脂肪细胞和皮下脂肪中表达无差别;ER及PR均为阴性.结论 毛囊周围基质的不同,可能影响囊壁的分化,FSCH实际是毛囊瘤分化的晚期阶段.
