血尿酸水平与冠脉狭窄及冠心病的关系

         

摘要

Objective To investigate the association between serum uric acid (SUA) levels and the presence of angiographic coronary artery stenosis and disease (IHD). Methods The study group consisted of 647 consecutive patients undergone coronary angiography. The anthropometric measurements including blood fat, and SUA were conducted for every patient. According to the results of angiography, the patients with or without normal coronary angiograms, or with or without CHD were divided into two groups. CAD was defined as stenosis 50% for at least one vessel. Results Under each division, the difference of SUA between two groups was insignificant in men, while significant in women ( P < 0. 0l ). SUA was not significantly associated with the presence of stenosis and CAD in men. In women, SUA was significantly associated with stenosis ( P < 0.05 ), although the association between SUA and CHD was not significant ( P > 0.05 ). However, after adjusting for traditional risk factors, the stepwise multivariate logistic analysis showed that the association was not significant ( P > 0.05 ). Conclusions Sex differences existed in SUA. In women, SUA is associated with CHD with statistical trend,and with stenosis significantly, but it is not an independent risk factor for stenosis. No such association is found in men.%目的 探讨血尿酸(SUA)水平与冠脉狭窄和冠心病(CHD)的关系.方法 选择647例行冠脉造影患者,根据其冠脉有无狭窄分为冠脉正常组和非正常组,以冠脉至少有1支狭窄≥50%分为CHD组与非CHD组.检测患者的血脂及SUA.结果 在冠脉狭窄和CHD两个分组标准下,男性不同年龄组间的SUA水平无统计学差异,女性有统计学差异(P<0.01).男性SUA水平与冠脉狭窄或CHD发生均无相关性;女性SUA水平与CHD发生虽无统计学差异,但与冠脉狭窄发生有关(P<0.05),对传统风险因素校正后,多因素分析显示女性SUA水平与冠脉狭窄无统计学意义(P>0.05).结论 SUA存在性别差异,女性高尿酸血症与CHD发生有一定关系,与冠脉狭窄密切相关,但非发生冠脉狭窄的独立危险因素.男性SUA水平与二者均无关.
