胃癌组织中 HIF-1α、STAT3的表达变化及其与患者预后的关系

             

摘要

目的:观察胃癌组织中缺氧诱导因子-1α(HIF-1α)、信号传导与转录活化因子3(STAT3)的表达变化及其与患者预后的关系。方法免疫组化染色法检测123例份胃癌根治术切除的胃癌组织和53例份正常胃黏膜组织中的HIF-1α和STAT3。结果胃癌组织中HIF-1α、STAT3的阳性表达率分别为64.2%(79/123)和62.6%(77/123),与正常胃黏膜组织的39.6%(21/53)和43.4%(23/53)相比,P均<0.05。 HIF-1α及STAT3的阳性表达与胃癌病理分化程度、肿瘤浸润深度、有无淋巴结转移、TNM分期及有无血管侵袭有关(P均<0.05), HIF-1α、STAT3共同阳性表达率随胃癌TNM分期的升高而升高(r=0.377,P<0.05)。胃癌组织中HIF-1α与STAT3的表达呈正相关(r=0.335,P<0.05)。 HIF-1α和STAT3阳性表达者术后5年生存率分别为21.9%和27.4%,阴性表达者分别为51.2%和41.5%,二者相比,P均<0.05。胃癌组织中HIF-1α阳性表达是影响胃癌患者预后的独立危险因素( RR=2.162,P<0.05)。结论胃癌组织中HIF-1α、STAT3的阳性表达率升高,二者的表达呈正相关,HIF-1α、STAT3阳性表达者的预后差,HIF-1α阳性表达和TNM分期对评估胃癌患者的预后有一定价值。%Objective To investigate the relationship between expression of hypoxia-inducible factor-1α(HIF-1α) and signal transducer and activator of transcription 3(STAT3) in gastric cancer tissue and analyze their prognostic value .Methods The ex-pression of HIF-1αand STAT3 in 123 gastric cancer patients who underwent radical resection and 53 cases of control with normal gas-tric tissue were examined by immunohistochemistry.Results The positive protein expression rates of HIF-1αand STAT3 in the gas-tric cancer tissue were 64.2%(79/123) and 62.6%(77/123), which were significantly higher than those in the normal tissue 39.6%(21/53) and 43.4%(23/53)(all P<0.05).The proteins expression of HIF-1αand STAT3 were in correlation with the degree of dif-ferentiation, TNM staging, lymph node metastasis, vessel invasion and invasion depth(P<0.05), and the co-expression rates of HIF-1αand STAT3 were correlated with the TNM stages(r=0.377, P<0.05).In gastric cancer tissue, the expression of HIF-1αwas positively correlated with the expression of STAT3 (r=0.335, P<0.05).The 5-year survival rate of patients with positive ex-pression of HIF-1αand STAT3 were 21.9%and 27.4%, which were significantly lower than those with negative expression(all P<0.05), whose 5-year survival rates were 51.2%and 41.5%.HIF-1αexpression were independent prognostic factors for gastric cancer (RR=2.162, P<0.05).Conclusions In gastric cancer tissue, the expressions of HIF-1αand STAT3 are elevated and positively correlated.Patients with positive expression of HIF-1αand STAT3, the prognosis are poor.Expression of HIF-1αand TNM staging are useful for predicting the prognosis of patients with gastric cancer .
