胃癌患者Vav1基因E59K位点多态性分布变化及意义

     

摘要

Objective To investigate the distribution changes of Vav1 gene E59K site polymorphism in the periphreal blood of patients with gastric cancer and its significance.Methods Totally 350 cases of patients with gastric cancer were selected as the gastric cancer group, and 200 cases of healthy people were recruited as the control group.The levels of CEA, FER and CA72-4 were detected in peripheral blood by chemiluminescence.The levels of Vav1 and IL-2 were tested with ELISA.PCR was applied to test E59K site polymorphism of Vav1 gene in the peripheral blood.The various indexes were compared between the two groups, and the relationships of each index with gastric cancer were analyzed.Results Compared with the control group, the levels of Vav1, CEA, FER and CA72-4 in the peripheral blood were higher, whereas the level of IL-2 was lower in the gastric cancer group (all P<0.05).Three kinds of genotypes AA, GA and GG were found for E59K site polymorphism of Vav1 gene.The frequencies of genotypes AA and GA and allelic gene A in the gastric cancer group were higher than those of the control group (all P<0.05).The level of IL-2 gradually decreased in genotypes GG, GA and AA, and the level of Vav1 gradually increased in genotypes GG, GA and AA in the gastric cancer group and the control group (all P<0.05).Logistic regression analysis showed that genotype AA was closely related with progress of gastric cancer, which was an independent risk factor for gastric cancer (OR=2.67, 95%CI:1.814-6.474, P=0.003). Conclusion In the Vav1 gene E59K site polymorphism of patients with gastric cancer, genotype AA is mainly found, which might promote the occurrence and development of gastric cancer by increasing Vav1 expression and decreasing IL-2 expression.%目的:探讨胃癌患者外周血Vav1基因E59 K位点多态性的分布变化及意义。方法选取350例胃癌患者作为胃癌组,同时取200例体检健康者作为对照组。化学发光法测定血清肿瘤标志物癌胚抗原( CEA)、铁蛋白(FER)、糖类抗原72-4(CA72-4)水平;ELISA法检测血清Vav1及IL-2水平;PCR法检测外周血Vav1基因E59K位点多态性的分布情况。对两组患者各指标进行比较,并就各指标与胃癌的关系进行Logistic相关分析。结果与对照组比较,胃癌组血清中Vav1、CEA、FER、CA72-4水平均升高,IL-2水平降低(P均<0.05)。外周血Vav1基因E59 K位点多态性有AA、GA、GG三种基因型。胃癌组中AA、GA的基因型频率及A等位基因频率均高于对照组( P均<0.05)。胃癌组和对照组按E59K基因型分组后发现IL-2的水平随GG基因型、GA基因型、AA基因型逐渐降低,Vav1的水平逐渐升高(P均<0.05)。 Logistic回归分析结果显示,AA基因型与胃癌关系密切,是胃癌的独立危险因素(OR=2.67,95%CI为1.814~6.474,P=0.003)。结论胃癌患者Vav1基因E59K位点多态性中,以AA基因型为主,该基因型可能通过升高Vav1、降低IL-2水平,从而促进胃癌发生发展。
