支气管哮喘患儿血浆H2S水平变化及意义

         

摘要

Objective To observe the changes of plasma hydrogen sulfide (H2S) levels in children with bronchial asthma and to explore its clinical significance.Methods A total of 76 children with bronchial asthma (acute exacerbation group) were divided into the mild acute exacerbation group (n =33),moderate acute exacerbation group (n =24),and severe acute exacerbation group (n =19) according to the condition of children admitted to hospital.Meanwhile,41 of children with asthma remission were selected as the asthma remission group and 50 healthy subjects as the control group.The lung function and induced sputum cell count in each group was measured.The level of plasma H2S was determined by UV Visible spectrophotometry,and the levels of TNF-α and IL-6 in the plasma were determined by enzyme-linked immunosorbent assay.Pearson product moment correlation was used to analyze the relationship between plasma H2S levels and other indexes.Results The lung function indicators of the acute exacerbation group were lower than those of the control group,asthma remission group (all P < 0.05).The percentage of total cells,percentage of neutrophils,percentage of eosinophils,plasma levels of TNF-α and IL-6 in the acute exacerbation group and asthma remission group were higher than those in the control group(P <0.05),while the percentage of macrophages and the plasma H2S level in the acute exacerbation group and asthma remission group were lower than those in the control group (both P < 0.05),and the changes of the above indicators were more significant in the acute exacerbation group (all P < 0.05).What's more,with the condition increase of bronchial asthma,the above indicators changed more obviously (P < 0.05).Among the children with acute exacerbation of bronchial asthma,the plasma H2S level was positively correlated with the lung function parameters and the percentage of macrophages (both P < 0.05),and were negatively correlated with the total cells,the percentage of neutrophils,percentage of eosinophils,and the levels of TNF-α and IL-6 (all P < 0.05).Conclusions The level of plasma H2S in children with bronchial asthma decreases.The more serious the conditions of bronchial asthma becomes,the lower the plasma H2S level is.%目的 观察支气管哮喘患儿血浆硫化氢(H2S)水平变化,并探讨其临床意义.方法 根据入院时病情将76例急性支气管哮喘患儿(急性发作期组)分为轻度急性发作期组(n=33)、中度急性发作期组(n=24)、重度急性发作期组(n=19),同期选取41例哮喘缓解期患儿为哮喘缓解期组、50例体检健康儿童为对照组.测定各组肺功能和诱导痰细胞计数,用紫外可见分光光度法测定血浆H2S水平,酶联免疫吸附法测定血浆TNF-α、IL-6水平.采用Pearson积矩相关分析血浆H2S水平与其他指标的关系.结果 急性发作期组肺功能指标水平较对照组、哮喘缓解期组低(P均<0.05).与对照组比较,急性发作期组、哮喘缓解期组诱导痰细胞总数及中性粒细胞、嗜酸性粒细胞百分比,血浆TNF-α、IL-6水平高,巨噬细胞百分比、血浆H2S水平低(P均<0.05),急性发作期组以上指标变化更明显(P均<0.05),且急性发作期患儿病情越严重,以上指标变化越明显(P均<0.05).支气管哮喘急性发作期患儿血浆H2S水平与肺功能指标、巨噬细胞百分比呈正相关(P均<0.05),与细胞总数、中性粒细胞百分比、嗜酸性粒细胞百分比、TNF-α、IL-6呈负相关(P均<0.05).结论 支气管哮喘患儿血浆H2S水平降低,且病情越严重其水平越低.
