目的:探讨乌司他丁注射液(U TI)辅助治疗70岁以上慢性阻塞性肺病急性加重期(AECOPD)的疗效和对患者外周血炎性因子和超敏C反应蛋白的影响.方法:将98例AECOPD患者随机分为对照组(49例)和治疗组(49例),对照组患者按照慢性阻塞性肺疾病急性加重期的治疗指南给予常规治疗,治疗组在常规治疗基础上通过微量泵加用乌司他丁注射液20万U,2次/d,连用7d后,采集动脉血进行血气分析,采外周静脉血测定炎性因子与CRP,判定两组的临床疗效.结果:治疗7d后,治疗组氧合指数为(289.2±25.3),显著高于对照组的(251.3±20.3)(u=8.179,P<0.05).治疗组总体有效率为89.8％,高于对照组的67.3％(P<0.05).治疗组治疗7d后外周血TNF-α,IL-6,IL-8均显著低于对照组(P<0.05).治疗组治疗7d后外周血CRP显著低于对照组(P<0.05).结论:AECOPD患者早期应用乌司他丁注射液能显著降低体内炎性因子及CRP的水平,能有效提高临床疗效.%Objective :To observe and explore the curative effect of ulinastatin (UTI) in the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) patients over 70 years of age and its influence on peripheral blood inflammatory factors .Methods :98 AECOPD patients were enrolled in the study and randomly divid-ed into the treatment group(49cases) and the control group(49cases) .Two groups of patients were given the use of anti-infective ,phlegm ,foundation treatment ,the treatment group was added with UTI injection by micro pump ,200 , 000 IU each time ,twice a day for 7d .Their arterial blood gas analysis and peripheral blood inflammatory factors were examined and the clinical efficacy was evaluated .Results :7 days after the treatment ,the oxygenation index was (289 . 2 ± 25 .3 )in the treatment group ,which was significantly higher than that in the control group (251 .3 ± 20 .3) ,(P<0 .05) .The total effective rate was 89 .8％ in the treatment group ,which was higher than that in the control group (67 .3％ ,P< 0 .05) .The inflammatory factors in peripheral blood in the treatment group were significantly lower than that in the control group .Conclusion :AECOPD patients with early application of UTI injection can significantly decrease inflammatory response ,correct disorders of cellular immune function ,and improve therapeutic efficacy .
