目的 探讨甘精胰岛素联合二甲双胍治疗2型糖尿病的临床疗效及安全性.方法 将126例口服降糖药物血糖控制不理想的2型糖尿病患者随机分为对照组(60例)与观察组(66例),对照组给予诺和灵N联合二甲双胍治疗,观察组给予甘精胰岛素联合二甲双胍治疗,疗程12周.比较两组患者血糖、血糖达标时间、糖化血红蛋白、胰岛功能及低血糖发生率.结果 观察组血糖达标时间显著低于对照组(P<0.05);观察组餐后2 h血糖及糖化血红蛋白显著优于对照组(P<0.05);观察组餐后2 h C肽值显著高于对照组(P<0.05);观察组低血糖发生率显著低于对照组(22.7% vs.41.7%,P<0.05).结论 与诺和灵N相比,甘精胰岛素联合二甲双胍降血糖效果更为理想,可改善胰岛功能,且安全性好.%Objective To explore the efficacy and safety of insulin glargine combined with metformin treatment for type 2 diabetes mellitus. Methods 126 cases who had poorly controlled blood glucose were randomly divided into control group and experience group. 60 patients of control group were given Novolin N and metformin treatment, and 66 cases of experience group received insulin glargine combined with metformin treatment. Blood glucose, time of reaching target of glucose, HbA1 c, islet function and accidence of hypoglycemia were compared between the two groups. Results Time of reaching target of glucose in experience group was much earlier than that in control group ( P <0. 05 );2 hPG, C-peptide 2 h after meal and HbA1 c in experience group were superior to those in control group with significant difference ( P < 0. 05 ); The accidence of hypoglycemia in experience group was much lower than that in control group( 22. 1% vs. 41. 1% ,P <0. 05 ). Conclusion Compared with Novolin N,insulin glargine combined with metformin for type 2 diabetes mellitus has better clinical efficacy, and it is benefit for improving islet function with good safety.
