补气生肌方促进糖尿病性难愈创面血管新生和创面修复愈合的作用机制

         

摘要

目的 观察补气生肌方对糖尿病大鼠难愈创面的血管生成和促进血管生成的因子表达的影响.方法 选取48只雄性SD大鼠,经高脂高糖饲料喂养1周后,按35 mg/kg腹腔注射1%链脲佐菌素(STZ)溶液,糖尿病模型造模成功后制作创面模型,随机分为观察组(24只)和对照组(24只),其中观察组局部应用补气生肌中药涂抹,对照组应用凡士林涂抹,均为每日1次.采用免疫荧光法检测血管生成情况,PCR检测创面组织中血管内皮细胞生长因子(Vascular endothelial growth factor,VEGF)和碱性成纤维细胞生长因子(Basic fibroblast growth factor,bFGF)的基因表达.结果 观察组第3天、第7天、第14天的创面愈合率高于对照组(P<0.05).免疫荧光检测结果显示,第7天观察组、对照组的创面组织CD-34阳性细胞数分别为34.39%±3.46%和12.42%±1.53%,两组比较差异有统计学意义(P<0.05).PCR检测结果显示,在第3天、第7天、第14天,观察组创面组织VEGF-A、bFGF mRNA表达高于对照组(P<0.05).结论 补气生肌方在治疗糖尿病难愈性创面时,可上调VEGF-A和bFGF基因表达,促进创面组织血管生成,有利于难愈性创面的愈合和重建.%Objective To observe the influence of Buqi Shengji formula on angiogenesis of diabetic refractory wounds and vascular endothelial growth factor.Methods SD male rats (n =48) were given intraperitoneal injection of 1% STZ 35 mg/kg after being fed with high-fat and high-sugar diet,and skin was scratched to establish the diabetic wound model after the diabetic model was established.The model rats were randomly divided into 2 groups.Observation group (n =24) was given Buqi Shengji formula,while control group (n =24) was given vaseline.The microvessel count in granulation tissue was determined by immunofluorescence,and the expression of VEGF and bFGF were determined by PCR.Results The wound healing rate in observation group was higher than that of control group on the 3rd,7th and 14th day (P < 0.05).The immunofluorescence results showed that the number of CD-34 positive cells was 34.39% ±3.46% in observation group and 12.42% ± 1.53% in control group respectively,and there was significant difference between the two groups (P < 0.05).The expression of VEGF-A and bFGF mRNA in the wound tissue of observation group was higher than that of control group on the 3rd,7th and 14th day (P < 0.05).Conclusion Buqi Shengji formula can up-regulate the expression of VEGF-A and bFGF gene in the treatment of diabetic refractory wounds,and promote the angiogenesis of wound tissue which is beneficial to the healing and reconstruction of refractory wounds.
