下颌下入路内镜辅助咽旁前间隙上区肿瘤切除的临床应用研究

摘要

目的 探讨下颌下入路内镜辅助切除咽旁间隙(PPS)上区肿瘤的适应证、禁忌症、手术技巧和优缺点.方法 回顾性分析2012年11月至2016年7月我科12例PPS前间隙上区肿瘤的手术资料及疗效.结果 12例患者咽旁前间隙上区肿瘤均完整切除,术区一期愈合,无面瘫及下唇麻木等并发症,2例恶性肿瘤患者术后放疗50 gY,12例患者随访9月～53月,无肿瘤复发.结论 下颌下入路内镜辅助手术适合切除咽旁前间隙上区的肿瘤,此术式安全可靠,结构显示清晰,手术创伤减少,避免了截骨带来的术后并发症,值得临床推广应用.%Objective To investigate the indications,contraindications,surgical techniques and advantages/disadvantages of endoscopically assisted dissection of parapharyngeal anterior upper space (PPS) tumor via submandibular approach.Methods The clinical and surgical data of 12 patients with PPS tumor were retrospectively analyzed.Results The PPS tumors of the 12 patients were completely removed.Surgery incision was healed well and no facial paralysis,lower limb numbness and other complications occurred.Two patients with malignant tumors were treated with 50 gY after surgery.Twelve patients were followed up from 9 to 53 months and no tumor recurrence was found.Conclusion The surgery of endoscopically assisted dissection of PPS tumor via submandibular approach is safe and reliable.It could clearly display structures,reduce surgical trauma and avoid the postoperative complications of the osteotomy.It is worthy of clinical application.
