目的：探讨氯吡格雷联合阿司匹林对短暂性脑缺血发作（ TIA）患者凝血功能的影响。方法选择安阳市中医院神经内科2011年1月—2014年1月收治的TIA患者115例，采用随机数字表法将患者分为对照组57例和试验组58例。对照组患者给予阿司匹林治疗，试验组患者给予氯吡格雷联合阿司匹林治疗，均连续治疗2周。观察治疗前后凝血指标变化，包括活化部分凝血活酶时间（ APTT）、凝血酶原时间（ PT）和纤维蛋白原（ FIB）；观察两组综合疗效及治疗期间不良反应发生情况。结果治疗前两组患者APTT、PT和FIB比较，差异无统计学意义（ P﹥0.05）；治疗后试验组患者 APTT 和 PT 为（36.75±4.62） s、（14.94±1.73） s，分别长于对照组的（30.73±4.56） s、（12.29±1.66）s，FIB低于对照组〔（2.10±0.33）g/L比（2.82±0.36）g/L〕（P﹤0.05）。试验组患者综合疗效优于对照组（P﹤0.05）。试验组患者不良反应发生率为5.2％，对照组为3.5％，差异无统计学意义（P﹥0.05）。结论氯吡格雷联合阿司匹林治疗TIA 效果确切，能够有效改善患者凝血功能。%Objective To investigate the impact of clopidogrel combined with aspirin on blood clotting function of transient ischemic attack( TIA) patients. Methods A total of 115 TIA patients were selected in Chinese Traditional Hospital of Anyang from January 2011 to January 2014,and they were divided into control group(n=57)and experiment group(n=58) according to random number table. Control group given aspirin,experiment group given clopidogrel combined with aspirin,both groups treated for 2 weeks. Activated partial thromboplastin time( APTT),prothrombin time( PT) and fibrinogen( FIB) were detected before and after treatment, and curative effect and incidence of adverse reactions during treatment were observed. Results No significant differences was found of APTT, PT and FIB between the two groups before treatment ( P ﹥0. 05 );APTT,PT,FIB of experiment group was(36.75 ±4.62)s,(14.94 ±1.73)s,(2.10 ±0.33)g/L,respectively,and APTT,PT were longer,FIB was lower than that of control group〔(30. 73 ± 4. 56) s,(12. 29 ± 1. 66) s,(2. 82 ± 0. 36) g/L,respectively〕(P﹤0. 05). The curative effect of experiment group was better than that of control group(P﹤0. 05);the incidence of experiment group was 5. 2%,that of control group was 3. 5%,the difference was not significantly different( P﹥0. 05). Conclusion Clopidogrel combined with aspirin has certain curative effect on TIA,can effectively improve the patients'blood clotting function.
