血管回声跟踪技术用于评价肥胖对青少年颈动脉弹性功能影响的价值

摘要

Objective To analyze the evaluation value of echo - tracking in evaluating the impact of obesity on carotid artery elastic function. Methods A total of 76 teenagers received physical examination were selected in the Friendship Hospital of Dalian from June 2013 to March 2014,and they were divided into A group(with BMI less than 24 kg/ m2 ,n = 38)and B group(with BMI equal or over 24 kg/ m2 ,n = 38)according to ″ Chinese School - age Overweight and Obesity Screening BMI Classification Criteria for Children and Adolescents″ enacted by WGOC. Echo - tracking was used to check the carotid artery elastic function,waistline,BMI,incidence of fatty liver,carotid intima - media thickness(CIMT)and index of carotid artery elastic function〔 including hardness index( β),pressure - elastic stress coefficient( Ep),arterial compliance( AC), augment index(AI)and pulse wave velocity( pWV)〕were compared between the two groups,and index of carotid artery elastic function was compared in teenagers with different genders. Results Waistline,BMI,CIMT and Ep of A group were statistically significantly larger than those of B group,incidence of fatty liver,β and AI of A group were statistically significantly higher than those of B group,AC of A group was statistically significantly lower than that of B group,while pWV of A group was statistically significantly faster than that of B group(P < 0. 05). Of the two groups,β of males was statistically significantly higher than that of females,Ep of males was statistically significantly larger than that of females,AC of males was statistically significantly lower than that of females,pWV of males was statistically significantly faster than that of females( P < 0. 05), while no statistically significant differences of AI was found between different genders(P > 0. 05). In males,β of A group was statistically significantly higher than that of B group,Ep of A group was statistically significantly lager than that of B group,AC of A group was statistically significant lower than that of B group,pWV of A group was statistically significantly faster than that of B group( P < 0. 05),while no statistically significant differences of AI was found between the two groups( P > 0. 05);in females,β of A group was statistically significantly higher than that of B group,Ep of A group was statistically significantly larger than that of B group,AC of A group was statistically significantly lower than that of B group,pWV of A group was statistically significantly faster than that of B group(P < 0. 05),while no statistically significant differences of AI was found between the two groups(P > 0. 05). pearson correlation analysis showed that,in teenagers with obesity,waistline,BMI,incidence of fatty liver was positively correlated with β,with Ep,with pWV,respectively( P < 0. 05),was negatively correlated with AC, respectively(P < 0. 05),but was not linearly correlated with AI(P > 0. 05). Conclusion The carotid artery elastic function of female teenagers with obesity is significantly worse than that of male teenagers,and echo - tracking can effectively evaluate the impact of obesity on carotid artery elastic function.%目的:分析血管回声跟踪(ET)技术用于评价肥胖对青少年颈动脉弹性功能影响的价值。方法选择2013年6月—2014年3月在大连市友谊医院进行体检的青少年76例,按照中国肥胖问题工作组制定的“中国学龄儿童青少年超重、肥胖筛查体重指数分类标准”将受试者分为体质量正常〔体质指数(BMI)<24 kg/ m2〕38例(正常组)和肥胖(BMI≥24 kg/ m2)38例(肥胖组)。采用 ET 技术检查两组受试者颈动脉弹性功能,比较两组受试者腰围、BMI、脂肪肝发生率、颈动脉内膜中层厚度(IMT)及颈动脉弹性功能指标〔包括硬度指数(β)、压力-应变弹性系数(Ep)、动脉顺应性(AC)、增大指数(AI)、脉搏波传导速度(pWV)〕,分层比较两组不同性别受试者间颈动脉弹性功能指标及两组相同性别受试者颈动脉弹性功能指标。结果肥胖组受试者腰围、BMI、颈动脉 IMT 及 Ep 大于正常组,脂肪肝发生率、β、AI 高于正常组,AC 低于正常组,pWV 快于正常组(P <0.05)。正常组和肥胖组男性受试者β高于女性,Ep 大于女性,AC 低于女性,pWV 快于女性(P <0.05);两组受试者不同性别间 AI 比较,差异无统计学意义(P >0.05)。肥胖组男性受试者β高于正常组男性,Ep 大于正常组男性,AC 低于正常组男性,pWV 快于正常组男性(P <0.05);两组男性受试者 AI 比较,差异无统计学意义(P >0.05)。肥胖组女性受试者β高于正常组女性,Ep 大于正常组女性,AC 低于正常组女性,pWV 快于正常组女性(P <0.05);两组女性受试者 AI 比较,差异无统计学意义(P >0.05)。pearson 相关性分析结果显示,肥胖组受试者腰围、BMI、脂肪肝发生率与β、Ep、pWV呈正相关(P <0.05),与 AC 呈负相关(P <0.05),与 AI 无直线相关性(P >0.05)。结论女性肥胖青少年颈动脉弹性功能较差,ET 技术能有效检出肥胖对青少年颈动脉弹性功能的影响。
