六君祛痰解毒汤联合 GP 化疗方案治疗非小细胞肺癌的临床疗效及对血清血管内皮生长因子水平的影响研究

摘要

目的：探讨六君祛痰解毒汤联合 GP 化疗方案治疗非小细胞肺癌的临床疗效及对血清血管内皮生长因子（VEGF）水平的影响。方法选取上海市徐汇区中心医院2011年1月—2013年8月收治的非小细胞肺癌患者148例，随机分为中药组49例、化疗组49例及联合组50例。中药组患者单纯采用六君祛痰解毒汤进行治疗，化疗组患者采用单纯 GP 化疗方案进行治疗，联合组患者采用六君祛痰解毒汤联合 GP 化疗方案进行治疗；21 d 为1个疗程，3组患者均治疗2个疗程。比较3组患者临床疗效、不良反应/毒副作用发生情况及治疗前后血清 VEGF 水平。结果化疗组和联合组患者临床疗效优于中药组（u 值分别为2.740、4.537，P 值分别为0.006、0.000）；化疗组与联合组患者临床疗效比较，差异无统计学意义（u =1.606，P =0.108）。3组患者治疗前血清 VEGF 水平比较，差异无统计学意义（P >0.05）；中药组和联合组患者治疗后血清 VEGF 水平低于化疗组，联合组患者治疗后血清 VEGF 水平低于中药组（P 0.05）。中药组患者未出现明显不良反应；联合组患者胃肠道反应、白细胞计数减少、贫血、血小板计数减少发生率均低于化疗组（P 0.05）。结论六君祛痰解毒汤联合 GP 化疗方案治疗非小细胞肺癌的临床疗效确切，能有效降低血清 VEGF 水平，且不增加不良反应/毒副作用发生风险。%Objective to observe the clinical effect of liujun qutan jiedu decoction combined with GP chemotherapy regimen on non - small cell lung cancer,to explore the impact on serum VEGF level. Methods From January 2011 to August 2013,a total of 148 patients with non - small cell lung cancer were selected in the Central Hospital of Xuhui District,Shanghai, and they were randomly divided into A group( n = 49),B group( n = 49) and C group( n = 50). Patients of A group received liujun qutan jiedu decoction,of B group received GP chemotherapy regimen( gemcitabine + cisplatin),of C group received liujun qutan jiedu decoction combined with GP chemotherapy regimen. All of the three groups treated for 2 courses(21 days as a course). Clinical effect,incidence of adverse reactions or toxic and side effects,serum VEGF levels before and after treatment were compared among the three groups. Results the clinical effect of B group and C group was statistically significantly better than that of A group(u = 2. 740,P = 0. 006;u = 4. 537,P = 0. 000),respectively while no statistically significant differences of clinical effect was found between B group and C group( u = 1. 606,P = 0. 108).No statistically significant differences of serum VEGF level was found among the three groups before treatment(P > 0. 05);serum VEGF level of A group and C group was statistically significantly lower than that of B group after treatment,respectively,that of C group was statistically significantly lower than that of A group after treatment(P 0. 05). No one of A group occurred any obvious adverse reactions;incidence of gastrointestinal reactions,decreased white blood cell count,anemia,decreased blood platelet count of C group was statistically significantly lower than that of B group,respectively( P 0. 05). Conclusion Liujun qutan jiedu decoction combined with GP chemotherapy regimen has certain clinical effect in treating non - small cell lung cancer,can effectively reduce the serum VEGF level without increasing the risk of adverse reactions or toxic and side effects.
