杨宝元教授自拟方加味治疗冠心病稳定型劳累性心绞痛的临床疗效

摘要

目的 观察杨宝元教授自拟方加味治疗冠心病稳定型劳累性心绞痛的临床疗效.方法 选取2015年6月—2017年3月在唐山市中医医院就诊的冠心病稳定型劳累性心绞痛60例,按照随机数字表法分为对照组和治疗组,每组30例.在常规对症治疗基础上,对照组患者给予通心络胶囊治疗,治疗组患者给予杨宝元教授自拟方加味治疗;两组患者均连续治疗4周.比较两组患者心电图改善效果、中医症候改善效果及综合疗效,治疗前后中医症候评分、血脂指标及治疗期间不良反应发生情况.结果 治疗组患者心电图改善效果、中医症候改善效果及综合疗效均优于对照组(P＜0.05).治疗前两组患者胸痛评分、胸闷评分、心悸评分、气促评分、疲倦乏力评分、舌象评分及脉象评分比较,差异无统计学意义(P＞0.05);治疗后治疗组患者胸痛评分、胸闷评分、心悸评分、气促评分、畏寒肢冷评分、舌象评分及脉象评分均低于对照组(P＜0.05).治疗前两组患者总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)水平比较,差异无统计学意义(P＞0.05);治疗后治疗组患者TC、TG及LDL-C水平低于对照组,HDL-C水平高于对照组(P＜0.05).治疗期间,两组患者均未出现明显不良反应,血、尿、便常规检查及肝肾功能检查均无异常.结论 杨宝元教授自拟方加味治疗能有效提高冠心病稳定型劳累性心绞痛患者心电图改善效果、中医症候改善效果、综合疗效,改善患者中医症候和血脂指标,且安全性较高.%Objective To observe the clinical effect of professor YANG Bao-yuan self-made modified Chinese herb decoction in treating coronary heart disease patients complicated with stable effort angina pectoris.Methods A total of 60 coronary heart disease patients complicated with stable effort angina pectoris were selected in the Traditional Chinese Medicine Hospital of Tangshan from June 2015 to March 2017,and they were divided into control group and treatment group according to random number table,each of 30 cases.Based on conventional symptomatic treatment,patients of control group received tongxinluo capsule,while patients of control group received professor YANG Bao-yuan self-made modified Chinese herb decoction;both groups continuously treated for 4 weeks.Improvement effect of electrocardiogram and TCM symptoms,comprehensive therapeutic effect,TCM syndromes score and blood lipids index before and after treatment,and incidence of adverse reactions was found between the two groups.Results Improvement effect of electrocardiogram and TCM symptoms,and comprehensive therapeutic effect of treatment group were statistically significantly better than those of control group (P ＜ 0.05).No statistically significant differences of chest pain score,chest distress score,palpitation score,anhelation score,extreme chilliness score,tongue picture score or pulse presentations score was found between the two groups before treatment (P ＞ 0.05),while chest pain score,chest distress score,palpitation score,anhelation score,extreme chilliness score,tongue picture score and pulse presentations score of treatment group were statistically significantly lower than those of control group (P ＜0.05).No statistically significant differences of TC,TG,HDL-C or LDL-C was found between the two groups before treatment (P ＞ 0.05);after treatment,TC,TG and LDL-C of treatment group were statistically significantly lower than those of control group,while HDL-C of treatment group was statistically significantly higher than that of control group (P ＜ 0.05).No one of the two groups occurred any obvious adverse reactions,abnormal blood,urine or stool routine examination results,or abnormal hepatic and renal function.Conclusion Professor YANG Bso-yuan self-made modified Chinese herb decoction can effectively improve the improvement effect of electrocardiogram and TCM symptoms,comprehensive therapeutic effect in treating coronary heart disease patients complicated with stable effort angina pectoris,is very helpful to relieve the TCM symptoms and adjust the blood lipids metabolism,with relatively high safety.
