对本院静脉配置中心(PIVAS)2011年1～12月抗肿瘤药物医嘱的审核,共发现不合理现象177次,主要表现在溶媒选择、给药剂量、给药顺序和途径方面.药师通过多种手段和方法,提供肿瘤专科药学服务,指导护士药品存放、正确使用共45人次；参与化疗全过程,防范不良反应的发生,提供出院教育123人次.%Medication orders of anticancer drugs through Pharmacy Intravenous Admixture Services (PIVAS) from Jan. to Dec. in 2011 were checked. The main problems of 177 irrational drug use were solvent selection, dosage, administration order and routes. Nurses were guided in 45 cases about drug store and proper use (flushing the piping after use, drip, etc.). Pharmacists took part in chemotherapy to prevent the occurrence of adverse reactions and provided education to 123 patients on discharge. Clinical pharmacists can provide pharmacy services on tumor to achieve safe, reasonable and effective drug treatment through various means and methods.
