骨骺滑动板对骨折愈合影响的实验研究

             

摘要

目的 设计一种骨骺部可滑动延长板,观察该钢扳对实验性山羊骨折愈合的影响.方法 根据幼年山羊股骨髁部的解剖特点设计出骨骺滑动板.动物选取月龄1.5月的健康山羊18只,雌雄各半,体重10~15Kg,右股骨采用普通钢板内固定,左股骨采用滑动板内固定.术后1个月、2个月、4个月3个时相点进行双侧股骨X线摄片检查,利用Image Pro Plus软件计算骨痂的平均灰度.术后2个月、4个月2个时相点各宰杀9只山羊,对双侧股骨标本行生物力学试验,了解骨痂强度.结果 骨痂X线灰度分析示术后1、2及4个月骨骺滑动板组骨痂形成面积和骨痂灰度均明显优于普通钢板组(P<0.05);生物力学试验显示骨骺滑动板组较普通钢板组在轴向刚度、三点弯曲、抗扭转三项测试指标在术后2个月、4个月均有较强的生物力学稳定性(P<0.05).结论 由于骨骺滑动板的独特设计使其在动物体内能够明显促进骨折愈合和增加有效骨痂的力学强度,可进行进一步的研究.%Objective To design a Slide-Traction Plate ,which can slide with the growth of epiphyses, and investigate the effects of this plate on the fracture healing, of experimental goat fracture. Methods The Slide-Traction Plate were designed for the femoral condyle configuration of the adolescent goat. 18 adolescent goats were enrolled in this study,Which were 1.5 -month-old,weighing 10-15Kg, half male and half female. On which the left femurs were fixed with Slide-Traction Plates as the experimental groups,and the right femurs were fixed with common plates as the control groups. All the goats were given X-ray examination at 1,2 and 4months after surgery.The X-ray extent of callus of both femurs were analysed through the Image Pro Plus system. And both femurs were harvested at 2 and 4 month after surgery with 9 animals in each time. The axial stiffness,bending stiffness and torsional stiffness were determined with biomechanical test to evaluate the bending strength of callus. Results The X-ray extent of callus in the experimental groups was hingher than that in the control groups at 1 month, 2 month, 4 month after surgery and there had significantly differences(P < 0.05) .The biomechanical tests showed that the biomechanical stability of the experimental groups were stronger than the control groups in anticompression,antibending and antitorsion at the same stage (P < 0.05 . Conclusion The Slide-Traction Plate, which has particular design particularly, can improve bonge healing and increase biomechanical stability of callus strength .It may be worth further study.
