前列腺穿刺活检标本中泡沫样腺癌的临床病理学特点及免疫组化染色表达特征

     

摘要

Objective To discuss the clinical pathological characteristics and immunohistochemical staining in biop-sy specimens of the foamy gland adenocarcinoma of the prostate. Method 30 cases of patients with foamy gland adeno-carcinoma of the prostate, which were confirmed by pathologic prostate biopsy, were included in the study, and the immu-nohistochemical expression, histomorphology characteristics, clinical features and prognostic follow-up of all patients were analyzed retrospectively. Result All tumors were characterized with irregular shape and size, or incomplete struc-ture of the abdominal gland in tumor tissues. The tumor cells were stained with foam, with abundant cytoplasm, which in-filtrated in benign prostatic glands. In these cells, there were no obvious nucleoli, while irregular nuclei and atypia cells were insignificant, with low and pyknotic karyoplasm. There was 6 cases with 2/10 tissues involvement, 12 cases of 3/8 tissues involvement, and 12 cases of 2/6 tissues involvement;12 cases had cancer cell expression of P504s, 18 cases were with cancer cell expression of AR and PSA, and all were diffuse expressions, though other markers, such as CK34be12, p63, S100, CD163, and CD68 were consistently negative. In positive control, basal cell were observed with expression of CK34be12 and p63. Conclusion Patients with foamy gland adenocarcinoma of the prostate are often observed with abundant and crowded foamy, cytoplasmic small glands, which are intermixed with normal benign glands with invasive growth.%目的 探讨前列腺穿刺活检标本中泡沫样腺癌的临床病理学特点及免疫组化染色表达特征.方法选择30例前列腺泡沫样腺癌患者,均经前列腺穿刺活检病理确诊,对所有患者的免疫组化表达谱、组织形态学特征、临床特征和预后随访情况进行回顾性分析.结果所有患者的肿瘤组织均表现为腺体大小、形状不规则或腺腔结构不完整.肿瘤细胞浅染呈泡沫状,具有丰富的细胞浆,于前列腺良性腺体间浸润.无明显核仁,核形不规则,异型不明显,核浆比例低、呈固缩状.累及2/10组织条者6例,累及3/8组织条者12例,累及2/6组织条者12例;癌细胞表达P504s者12例,癌细胞表达雄激素受体(AR)、前列腺特异抗原(PSA)者18例,均为弥漫性表达,其他标志物如CK34be12、p63、S100、CD163、CD68等均呈阴性.阳性对照(前列腺良性腺体)的基底细胞表达CK34be12、p63.结论前列腺泡沫样癌患者伴有丰富且互相拥挤的泡沫状胞质小腺体,在正常的良性腺体中混杂呈浸润性生长.
