目的：探讨子宫动脉栓塞术配合清宫治疗剖宫产术后子宫瘢痕妊娠的疗效与护理，以期提高临床诊治水平。方法：选取2011年1月～2013年1月126例剖宫产术后子宫瘢痕妊娠患者并随机等分为对照组和观察组，对照组给予子宫动脉栓塞术治疗；观察组给予子宫动脉栓塞术配合清宫治疗，并加强护理干预，观察治疗前后相关指标变化情况。结果：观察组患者腹痛、胃肠道反应、阴道不规则流血、复发发生率均低于对照组，再次正常妊娠率高于对照组，两组比较差异具有统计学意义（P＜0．05）。观察组血清β－HCG恢复正常时间、阴道出血干净时间、子宫下段异常回声消失时间、月经恢复正常时间、住院时间均短于对照组（均P＜0．05）；观察组治疗后3，7，14 d血清β－HCG下降较对照组明显（ P＜0．05）。结论：子宫动脉栓塞术配合清宫治疗剖宫产术后子宫瘢痕妊娠临床效果显著，加强护理能进一步巩固疗效，降低并发症发生率。%Objective:To investigate the curative effect and nursing of treatment of cesarean scar pregnancy through uterine artery embolization combined with curettage in order to improve the level of clinical diagnosis and treatment .Methods:Selected 126 cases of patients with cesarean scar pregnancy dated from January 2011 to January 2013 and randomly divided them into control group and observation group .The control group was given uterine artery emboliza-tion treatment,while the observation group was given uterine artery embolization combined with curettage and strengthen the nursing intervention .Index changes before and after treatment were observed.Results:Patients with abdominal pain,gastrointestinal reaction,irregular vaginal bleeding and the recur-rence rate of observation group were lower than that of the control group,while the another normal pregnancy rate was higher than that of the control group. The difference was of statistically significance (P<0.05).The serum β-HCG recovery time,vaginal bleeding cleaning time,the lower uterine segment ab-normal echo disappearance time, menstrual recovery time and hospitalization time of observation group was shorter than the control group (all P<0.05);the serum β-HCG of observation group after 3,7 and 14 days of treatment decreased significantly compared with the control group (P<0.05).Conclu-sion:There are significant clinical effect of uterine artery embolization combined with curettage in the treatment of cesarean scar pregnancy .Given strengthen nursing can further consolidate curative effect and reduce the incidence of complications .
