动脉瘤栓塞联合脑脊液置换术治疗动脉瘤引起的蛛网膜下腔出血的疗效及安全性

摘要

目的:探究与分析探究动脉瘤栓塞联合脑脊液置换术治疗动脉瘤引起的蛛网膜下腔出血(SAH)的疗效及安全性.方法:选取因动脉瘤引起的SAH患者96例,采取随机数字表法分为栓塞组与联合组,各48例.栓塞组仅给予介入栓塞术治疗,联合组给予脑脊液置换术联合介入栓塞术治疗,对比2组临床疗效、头痛持续时间、脑脊液恢复正常时间、颈强直持续时间、住院时间、并发症发生率及死亡率.结果:栓塞组总有效率为62.50％,联合组总有效率为89.58％,联合组总有效率明显高于栓塞组(P＜0.05).与栓塞组相比,联合组头痛持续时间、脑脊液恢复正常时间、颈强直持续时间、住院时间均缩短,脑积水率、脑血管痉挛率均较低(P＜0.05).2组死亡率、再出血率差异无统计学意义(P＞0.05).结论:动脉瘤栓塞联合脑脊液置换术治疗动脉瘤引起的SAH的临床疗效显著,可缩短症状缓解时间,降低并发症发生率,安全性较高.%Objective:To explore and analyze the efficacy and safety of aneurysm embolization combined with cerebrospinal fluid replacement for subarachnoid hemorrhage (SAH) caused by aneurysm.Methods:Ninety-six cases of SAH caused by aneurysm were randomly divided into control group and observation group (n=48 respectively).The cases in the control group were treated with interventional embolization,while the cases in the observation group were given cerebrospinal fluid replacement combined with interventional embolization.The efficacy,the time duration of headache,cerebrospinal fluid returning to normal,neck stiffness,hospital stay,complications and mortality rate were compared between the 2 groups.Results:The total effective rate of the control group was 62.50％,significantly lower than that of the observation group 89.58％ (P＜0.05).The time duration of headache,cerebrospinal fluid return to normal,neck stiffness,hospital stay of the observation group were shorter than those of the control group (P＜0.05).The rate of cerebral vasospasm and hydrocephalus in the operation group were significantly lower than those in the control group (P＜0.05).There were no statistically differences in mortality and rebleeding rate between the 2 groups (P＞0.05).Conclusion:It is an effective and safe way to treat SAH caused by aneurysm with aneurysm embolization combined with cerebrospinal fluid replacement.
