经颅多普勒检测颅脑损伤后早期脑血流动力学变化研究

         

摘要

Objective IL is Lo sLudy Lhe funcLion and value oi' Lranscranial Doppier( TCD ) in Lhe deLecLion oi' cerebral blood How in mild or moderaLe craniocerebral injury. Methods 40 cases oi' paLienLs wiLh mild or moderaLe craniocerebral injury were chosen as observation group, and 40 cases oi healthy people were chosen as control group. The indexes oi cerebrovascular hemodynamics were detected by TCD in both groups, and Lhe detected results were analyzed with statistical methods. Results In Lhe observation group, Lhere were 35 cases ( 88% ) whose cerebral flow were abnormal, in which 28 cases ( 80% ) wiLh rapid speed oi' cerebral blood flow , buL compared wiLh Lhe conLrol group, Lhe difference was noL significanL ( P > 0. 05 ) ; Lhere were 7 cases ( 20%' ) wiLh slow cerebral blood ilow and 14 cases ( 35%' ) wiLh high pulsaLiliLy index. Conclusion TCD can be used as a eiiecLive method ior detecting cerebrovascular hemodynamics aiLer mild or moderaLe craniocerebral injury in early sLage, and iL can provides direcL and reliable evidence for Lhe early inLervenLion and LreaLmenL for this injury in clinic.%目的 研究经颅多普勒(TCD) 检测轻、中型颅脑损伤患者早期脑血流动力学变化的作用及价值.方法 选择40例急性轻、中型颅脑损伤患者作为观察组,40例健康者作为对照组.应用TCD分别检测2组的脑血流动力学指标,对检测结果 进行统计学处理.结果 观察组TCD检测脑血流异常35例(88%),其中28例(80%)表现为脑动脉血流速度增快,但与对照组相比无显著性差异(P>0.05);7例(20%)表现为血流速度减慢.14例(35%)出现血流搏动指数增高.结论 TCD可以作为轻、中型颅脑外伤后早期脑血流动力学变化的有效检测方法,能够为临床早期干预治疗提供直接可靠的依据.
