参黄散敷脐联合热敏灸疗法治疗大肠部分切除术后排便功能障碍疗效观察

摘要

目的 观察参黄散敷脐联合热敏灸疗法对大肠部分切除术后患者排便功能障碍的影响.方法 将56例大肠部分切除术患者随机分为观察组和对照组.对照组采用临床常规治疗,观察组在此基础上给予参黄散敷脐联合热敏灸疗法治疗,2组均以1周为1个疗程,治疗1个疗程后,观察2组患者排便功能障碍改善情况,检测治疗前后血清胃动素和生长抑素含量.结果 观察组治疗后排便功能总有效率显著高于对照组(P＜0.05);观察组治疗后肠鸣音恢复时间、不规则腹痛消失时间、首次肛门排气时间以及首次肛门排便时间均显著短于对照组(P均＜0.05);2组治疗后肠鸣音评分、腹胀痛评分及胃肠反应评分均显著降低(P均＜0.05),且观察组各评分均显著低于对照组(P均＜0.05);治疗后,2组血清胃动素含量均显著升高(P均＜0.05),生长抑素含量均显著降低(P均＜0.05),且观察组治疗后血清胃动素和生长抑素含量改善情况均显著优于对照组(P均＜0.05).结论 参黄散敷脐联合热敏灸疗法能够有效改善大肠部分切除患者术后排便功能障碍,促进患者胃肠道功能恢复,安全可靠,值得临床推广.%Objective It is to observe the effect of Shenhuang powder by application on navel with thermal moxibustion on defecation dysfunction after large intestine partial resection.Methods 56 patients with defecation dysfunction after large intestine partial resection were randomly divided into observation group and control group.The control group was treated with clinical routine treatment,the observation group was treated with Shenhuang powder by application on navel with thermal moxibustion based on the therapy in the control group.One week as one treatment course,and after one course,the improvement of defecation dysfunction was observed and the contents of serum motilin and somatostatin before and after treatment were determined in both groups.Results The total effective rate of defecation function was significantly higher in the treatment group than that in the control group (P ＜ 0.05).After treatment,the time of bowel sound recovery,irregular abdominal pain disappearing,the first anal aerofluxus and defecation were significantly shorter in the observation group than that in the control group (P ＜ 0.05);the scores of bowel sounds,abdominal distension and pain and gastrointestinal reaction were significantly reduced in both groups(P ＜ 0.05),and the improvements of these scores in the observation group were significantly better than those in the control group (P ＜ 0.05);After treatment,the content of serum motilin was significantly increased while that of somatostatin was decreased in both groups(P ＜ 0.05),and the improvements of these indexes were better in the observation group than that in the control group (P ＜ 0.05).Conclusion Shenhuang powder by application on navel with thermal moxibustion can effectively improve postoperative defecation dysfunction,promote the recovery of gastrointestinal function with good safety and reliability in the patients after large intestine partial resection,it is worthy of clinical promotion.
