条口透刺承山穴结合推拿手法对肩周炎患者肩关节功能和疼痛的影响

摘要

目的 观察条口透刺承山穴结合推拿手法对肩周炎患者肩关节功能和疼痛的影响.方法 将62例肩周炎患者随机分为观察组和对照组各31例.对照组给予常规治疗,如吲哚美辛肠溶片口服及肩部功能锻炼等,观察组在对照组基础上联合条口透刺承山穴结合推拿手法治疗,共治疗4周.观察2组治疗前后疼痛[疼痛分级指数(PRI)、视觉模拟疼痛评分(VAS)和现在疼痛状况(PPI)]、肩关节功能及实验室指标[白细胞介素-10(IL-10)、血浆P物质(SP)、肿瘤坏死因子-α(TNF-α)、前列腺素E2(PGE2)]的变化情况,并评估临床疗效.结果 观察组治疗后总有效率明显高于对照组(P＜0.05);2组治疗后PRI评分、VAS评分、PPI评分及总分均明显降低(P均＜0.05),且观察组治疗后各评分及总分均显著低于对照组(P均＜0.05);2组治疗后肩关节功能各项评分及总分均明显升高(P均＜0.05),且观察组治疗后肩关节功能各项评分及总分均显著高于对照组(P均＜0.05);2组治疗后IL-10水平均明显升高(P均＜0.05),SP、TNF-α、PGE2水平均明显降低(P均＜0.05),且观察组治疗后上述指标改善情况均明显优于对照组(P均＜0.05).结论 条口透刺承山穴结合推拿手法可明显改善肩周炎患者肩关节功能障碍,减轻疼痛.%Objective It is to observe the effect of penetration acupuncture from Tiaokou to Chengshan acupoints combined with manipulation on shoulder joint function and pain in patients with frozen shoulder.Methods 62 patients with frozen shoulder were randomly divided into observation group and control group,with 31 cases in each group.The control group was treated with conventional therapy such as Indomethacin plus exercise of shoulder function,while the observation group was treated with penetration acupuncture from Tiaokou to Chengshan acupoints combined with manipulation based on the therapy in the control group.Both groups were treated for 4 weeks.The changes of pain [(graded pain index (PRI),visual analog pain score (VAS) and current pain status (PPI)],shoulder joint function and laboratory parameters [IL-10,P substance (SP),tumor necrosis factor-or (TNF-α),prostaglandin E2 (PGE2)] before and after treatment were observed in both groups,and the clinical efficacy was evaluated.Results The total effective rate of the observation group was significantly higher than that of the control group (P ＜ 0.05).The PRI scores,VAS scores,PPI scores and total scores of the two groups were significantly decreased after treatment (P ＜ 0.05),and these scores in the observation group were significantly lower than those in the control group (all P ＜ 0.05).The scores and total scores of shoulder joint function were significantly increased in both groups after treatment(P ＜ 0.05),the scoring and total scores of shoulder joint function in the observation group were significantly higher than those of in the control group (all P ＜ 0.05).The levels of IL-10 in the two groups were significantly increased (P ＜0.05),and the levels of SP,TNF-α,PGE2 were significantly decreased in both groups (all P ＜ 0.05).The improvements of the above indexes in the observation group was better than that in the control group (all P ＜ 0.05).Conclusion Penetration acupuncture from Tiaokou to Chengshan acupoints combined with manipulation can significantly improve the shoulder joint dysfunction and relieve pain in patients with frozen shoulder.
