照顾者反应对温州市某医院原发性肝癌患者自我感受负担的影响

摘要

目的：探讨照顾者反应对原发性肝癌患者自我感受负担（ SPB ）的影响。方法：采用中文版照顾者反应评估量表和SPB量表对223例原发性肝癌患者及其主要照顾者进行问卷调查，比较不同程度SPB的肝癌患者照顾者反应的差别，采用Pear-son相关分析法和分层回归分析法探讨照顾者反应对肝癌患者SPB的影响。结果：不同程度SPB组的健康问题、时间安排受打扰、经济问题、家庭支持等方面缺乏评分比较，差异均有统计学意义（均P＜0．05）； Pearson相关分析显示，肝癌患者SPB与照顾者反应中的健康问题（r＝0．362，P＝0．018）、时间安排受打扰（r＝0．329，P＝0．029）、经济问题（r＝0．341，P＝0．021）、家庭支持缺乏（r＝0．389，P＝0．007）均显著正相关；与自尊显著负相关（r＝-0．375，P＝0．022）。分层回归分析显示，家庭收入、疾病分期和照顾者反应是肝癌患者SPB的影响因素。结论：原发性肝癌患者照顾者反应较差，这可能是原发性肿癌患者SPB的影响因素。%Objective: To explore the effect of caregiver reaction on the self-perceived burden ( SPB) in patients with primary hepatic car-cinoma (PHC).Methods:223 patients with PHC and their caregiver were questionnaire by the Chinese version caregiver reaction assessment scale and self-perceived burden scale.The score of caregiver reaction was compared between the PHC patients with different levels of SPB . Pearson correlation analysis and hierarchical regression analysis were used to explore the effect of caregiver reaction on the SPB in patients with PHC.Results: there's significant difference of the score of caregiver reaction in the PHC patients with different levels of SPB(P<0.05);Pearson correlation analysis showed that the score of SPB was significantly positive correlated with health problems (r=0.362, P=0.018), time arrange-ments disturbed (r=0.329, P=0.029), economic problems (r=0.341, P=0.021), lack of family support (r=0.389, P=0.007),but sig-nificantly negative correlated with self-esteem(r=-0.375,P =0.022); Hierarchical regression analysis showed that family income, disease stage and caregiver reaction were the influence factors of SPB in patients with PHC .Conclusions:The poor caregiver reaction may be an impor-tant reason for the SPB in patients with PHC.
