加巴喷丁治疗顽固性痛性周围神经病的疗效观察

摘要

Objective To discuss the curative effect of Gabapentin for the intractable painful peripheral neuropathy caused by various factors. Methods A retrospective analysis was done for 86 patients who suffered painful peripheral neuropathy according to the diagnostic conclusions reached by our Outpatient Department and Inpatient Department during the period from January 2009 to August 2010. The 86 patients were divided into a nondiabetic group ( 46 ) and a diabetic group ( 40 ). All the said patients were treated with Gabapentin. The degree of relief of the symptoms was the main indicator for the evaluation of the curative effect. Results The curative effect of the nondiabetic group was much better than that of the diabetic group.Conclusion Gabapentin is able to improve the intractable pain of peripheral neuropathy, particularly PHN,intractable TN, the peripheral neuralgia caused by chronic alcoholism, etc. Due to its different pathogenesis, the diabetic peripheral neuralgia needs more researches for its curative effect.%目的 探讨加巴喷丁对各种原因所致的顽固性痛性周围神经病的治疗效果.方法 对我院2009年1月至2010年8月门诊及住院部诊断为顽固性痛性周围神经病且给予加巴喷丁治疗的共86例患者进行分析.将86例患者分为非糖尿病组及糖尿病组,给予加巴喷丁治疗,以症状缓解程度为评价的主要疗效指标.结果 治疗后,非糖尿病与糖尿病组麦吉疼痛评分、治疗后生活质量影响、患者的评价间差异有统计意义(P<0.05);两组睡眠影响程度间差异无统计学意义.结论 加巴喷丁可改善不伴有糖尿病周围神经病的顽固性疼痛,尤其带状疱疹后遗神经痛、顽固性三叉神经痛、慢性酒精中毒性周围神经痛等.因糖尿病性周围神经病疼痛发病机制的不同,治疗上还需不断探索.
