腹腔镜下子宫肌瘤切除术60例临床分析

摘要

Objective To discuss the technical features and precautions of laparoscopic myomectomy ( LM ). Methods Clinical data of 60 cases of LM during Jan. 2006 to Dec. 2009 in our hospital were retrospectively reviewed,, and the method and curative effect were summarized and analyzed. Results Laparoscopic operation of all the 60 cases were successful,no conversion to open surgery case,no intraoperative and postoperative complications. Average operation time 30-140 min( 68.41 ±38. 20 )min. Intraoperative bleeding volume 20-130 mL, average( 49. 2 ± 31. 6 )mL, good operation effect were achieved. Muscle intramural fibroids surgery time was longer than subserosal fibroids operation, and the bleeding amount was more too. Surgery time of single fibroid with diameter bigger than 4 cm was longer than the ones with diameter smaller than 4 cm,and the bleeding amount was more too,difference with statistical significance( P <0.05 ). Conclusion LM is an ideal mode of operation for uterine fibroids,with affirmative efficacy,and worthy of promotion.%目的 探讨腹腔镜下子宫肌瘤切除术(LM)的技术特点及注意事项.方法 回顾分析2006年1月至2009年12月在我院经腹腔镜行子宫肌瘤切除的60例患者的临床资料,并对其治疗方法及疗效等进行总结分析.结果 60例患者均在腹腔镜下手术成功,无中转开腹病例,无术中及术后并发症发生.手术时间30 ～140 min,平均(68.41±38.20)min,术中出血量20 ～130 mL,平均(49.2±31.6)mL,取得了较好的手术效果.肌壁间肌瘤较浆膜下肌瘤手术时间长,出血量多;单个肌瘤直径＞4 cm较＜4 cm者手术时间长,出血量多,差异有统计学意义(P＜0.05).结论 LM是子宫肌瘤理想的手术方式,疗效确切,值得临床推广.
