中药复方罗布麻片联合胺碘酮治疗冠心病室性心律失常的临床效果分析

摘要

目的：观察中药复方罗布麻片联合胺碘酮治疗冠状动脉粥样硬化性心脏病(冠心病)室性心律失常的临床疗效。方法将2009年2月至2012年4月在湖北医药学院附属东风医院住院治疗的125例冠心病室性心律失常患者依据随机数字表法分为3组：联合治疗组(45例)采用中药复方罗布麻片结合胺碘酮治疗,复方罗布麻片每日3次,每次2片,胺碘酮第1周每日3次,第2周每日2次,第3周每日1次,每次均为200 mg；胺碘酮组(45例)采用胺碘酮治疗,方法同联合治疗组；对照组(35例)采用普罗帕酮治疗,每次150 mg,每日3次。3组患者疗程均为8周。比较3组患者的临床疗效。结果治疗后,联合治疗组总有效率为95．6%(43/45),胺碘酮组为86．7%(39/45),对照组为74．3%(26/35),3组比较差异有统计学意义(P<0．05)；3组治疗后的室性期前收缩数、短阵室性心动过速(短阵室速)数均呈现明显下降的趋势,而联合治疗组、胺碘酮组的室性期前收缩数、短阵室速数均低于对照组,联合治疗组的室性期前收缩数、短阵室速数低于胺碘酮组,差异均有统计学意义(均P<0．05)。结论中药复方罗布麻片联合胺碘酮治疗冠心病室性心律失常疗效显著,用药安全,价格低廉,用药途径方便。%Objective To observe the clinical curative effect of traditional Chinese medicine ( TCM ) compound kendir leaves tablets combined with amiodarone in the treatment of ventricular arrhythmias coro-nary heart disease. Methods Total of 125 patients with ventricular arrhythmias coronary heart disease admitted to the Affiliated Dongfeng Hospital of Hubei Medical College from Feb. 2009 to Apr. 2012 were ran-domly divided into three groups:combined treatment group ( 45 cases ) were treated with TCM compound kendir leaves tablets combined with amiodarone therapy,compound kendir leaves tablets 3 times a day,2 tab-lets each time,3 times daily amiodarone first week,second week 2 times a day,third weeks,1 time a day, each time 200 mg;amiodarone group(45 cases) were treated with amiodarone therapy,methods with the com-bined treatment group;control group (35 cases) were treated with propafenone,150 mg each time,3 times a day. All of the 3 groups of patients were treated for 8 weeks,and the clinical efficacy of the 3 groups was com-pared. Results After treatment,the total effective rate of the combined treatment group was 95. 6% (43/45),of the amiodarone group was 86. 7% (39/45),of the control group was 74. 3% (26/35),the differ-ences were statistically significant(P<0. 05). After treatment,premature ventricular number,nonsustained ventricular tachycardia of the 3 groups were obviously decreased,while ventricular premature number,nonsus-tained ventricular tachycardia of the combined treatment group and the amiodarone group were lower than those in the control group,the difference was statistically significant(P<0. 05),premature ventricular num-ber,nonsustained ventricular tachycardia of the combined treatment group were less than the amiodarone group,the difference was statistically significant(P<0. 05). Conclusion The efficacy of TCM compound kendir leaves tablets combined with amiodarone therapy in treatment of coronary heart disease ventricular arrhythmia is satisfactory,safe,inexpensive,and the route of administration is convenient.
