流量连续气道正压通气呼吸机治疗新生儿肺透明膜病临床研究

         

摘要

目的 观察流量连续气道正压通气(CPAP)呼吸机治疗新生儿肺透明膜病的临床疗效.方法 选取2010年6月至2012 年2月来东莞市黄江医院重症监护室住院治疗的46 例新生儿肺透明膜病患儿作为受试对象,采用随机数字表法分为两组,观察组(26例)采用流量CPAP呼吸机治疗,对照组(26例)接受肺表面活性物质治疗,对比两组治疗后的临床疗效、血气指标、肺功能变化. 结果 治疗后,观察组患者的血气指标动脉血氧分压显著高于对照组[(58.4 ±4.7) mmHg (1 mmHg =0.133 kPa) 比(53.6 ±5.7) mmHg,P <0.05],动脉血二氧化碳分压显著低于对照组[(34.2 ±3.8) mmHg 比(39.3 ±3.8) mmHg,P <0.05],观察组用力肺活量.1 s 用力呼气量?呼气流量峰值高于对照组[(2.75 ±0.46) L 比(2.06 ±0.33) L,(2.42 ±0.46) L 比(1.89 ±0.37) L,(4.41 ±0.56) L/s 比(3.83 ±0.44) L/s,P <0.05 或P <0.01],且观察组总有效率为95.6%,对照组总有效率为73.9%,两组疗效比较差异有统计学意义(P <0.05).结论 流量CPAP 呼吸机治疗新生儿肺透明膜病创伤小、起速快.%Objective To observe the clinical effect of the flow of continuous positive airway pressure ( CPAP) ventilation on neonatal pulmonary hyaline membrane disease.Methods A total of 46 cases of neo-natal pulmonary hyaline membrane disease admitted in ICU of Huangjiang Hospital from Jun .2010 to Feb. 2012 were divided into two groups by random number table method, patients in the observation group (26 cases) treated with CPAP ventilator,while patients in the control group were treated with pulmonary sur-factant.After treatment, changes of the clinical efficacy, blood gas indexes, lung function between the two groups were compared.Results After treatment,partial pressure of arterial partial pressure of oxygen in the observation group was higher than that in the control group[(58.4 ±4.7) mmHg vs (53.6 ±5.7) mmHg, P <0.05 ] , while arterial partial pressure of carbon dioxide was lower than that in the control group [(34.2 ±3.8) mmHg vs (39.3 ±3.8) mmHg,P<0.05].In the observation group,levels of forced vital capacity,forced expiratory volume in 1 second,peak expiratory flow were higher than those in the control group[(2.75 ±0.46) L vs (2.06 ±0.33) L,(2.42 ±0.46) L vs (1.89 ±0.37) L,(4.41 ±0.56) L/s vs(3.83 ±0.44) L/s,P<0.05 or P<0.01].The total effective rate of the observation group was 95.6%, which was significantly higher than 73.9% of the control group(P<0.05).Conclusion The flow of CPAP ventilation in treating neonatal pulmonary hyaline membrane disease has the advantages of little trauma ,fast action,and little side effect.
