摘要

Objective To explore the surgery effect of ultrasonic knife and postoperative recovery for the patients with breast cancer.Methods From May 2012 to May 2015,328 patients with breast cancerin Cen-tral Hospital of Zaozhuang Mining Group for surgical treatment were included in the study and divided into an observation group and a control group according to the drawing lot method,164 cases each.The control group was given electric knife and other conventional adjuvant therapy for treatment;the observation group was giv-en ultrasonic knife instead of electric knife on the basis of the control group′s treatment.The 24 h drainage, total drainage,drainage time,intraoperative blood loss,operative time,bed time,the cases of subcutaneous fluid,skin flap necrosis cases,the cases of upper limb edema and overall satisfaction of the two groups after operation were compared.And the relapse rate and quality of life score were detected 6 month after operation. Results Blood loss,operative time,bed time,24 h drainage,total drainage,drainage time,the subcutaneous fluid cases,the skin flap necrosis cases and the upper limb edema cases of the observation group were lower than the control group[(56 ±14) mL vs (74 ±22) mL,(104 ±16) min vs (133 ±22) min,(5.9 ± 1.8) d vs (6.6 ±2.1) d,(144 ±26) mL vs (165 ±32) mL,(289 ±47) mL vs (307 ±51) mL,(15 ± 4) d vs (16 ±5) d,4.27%(7/164) vs 10.98%(18/164),0.61%(1/164) vs 10.98%(18/164), 7.32%(12/164) vs 15.85%(26/164)],the total satisfaction of the observation group was higher than the control group[97.56%(160/164) vs 92.68%(152/164)],the differenced wrtr statistically significant (P<0.05);6 months after operation,the recurrence rate of the observation group was lower than the control group[1.22%(2/164) vs 5.49%(9/164)] and the quality of life score of the observation group was higher than the control group[(47.03 ±3.51) scores vs (45.98 ±4.72)scores],the differences were statistically significant( P <0.05 ) .Conclusion By using ultrasonic knife instead of electric knife can significantly shorten the time of breast cancer surgery,reduce blood loss and postoperative complication rate,at the same time it also can improve the therapeutic effect of radical surgery and postoperative quality of life .%目的：探究超声刀对乳腺癌患者手术效果及术后恢复情况的影响。方法选取2012年5月至2015年5月枣庄矿业集团中心医院就诊的需进行手术治疗的乳腺癌患者328例，依据抽签法分为观察组和对照组，各164例。对照组给予常规电刀及相关手术治疗，观察组给予超声刀代替电刀进行手术治疗；比较两组患者术后24 h引流量、总引流量、引流时间、术中出血、手术时间、术后卧床时间、皮下积液例数、皮瓣坏死例数、上肢水肿例数、总满意度及术后6个月复发率和生存质量评分。结果观察组患者的术中出血量、手术时间、术后卧床时间、术后24 h引流量、总引流量、引流时间、皮下积液、皮瓣坏死及上肢水肿发生率显著低于对照组[（56±14） mL 比（74±22） mL，（104±16） min比（133±22） min，（5．9±1．8） d比（6．6±2．1） d，（144±26） mL比（165±32） mL，（289±47） mL比（307±51） mL，（15±4） d比（16±5） d，4．27％（7／164）比10．98％（18／164），0．61％（1／164）比10．98％（18／164），7．32％（12／164）比15．85％（26／164）]，观察组总满意度显著高于对照组[97．56％（160／164）比92．68％（152／164）]，差异有统计学意义（P＜0．05）；术后6个月观察组患者的复发率显著低于对照组[1．22％（2／164）比5．49％（9／164）]，观察组患者的生存质量评分显著高于对照组[（47．03±3．51）分比（45．98±4．72）分]，差异有统计学意义（P＜0．05）。结论应用超声刀替代普通电刀可明显缩短乳腺癌患者手术时间，降低术中出血及术后并发症发生率，还可提高根治性手术的治疗效果及患者术后生存质量。
