硫辛酸治疗早期糖尿病足的临床疗效及对氧化应激反应和炎症指标的影响

         

摘要

目的:探讨硫辛酸治疗早期糖尿病足的临床疗效以及对氧化应激反应和炎症指标的影响。方法选择2014年1月至2015年3月在中国石化集团胜利石油管理局胜利医院接受治疗的早期糖尿病足患者120例,依据随机数字表法分为观察组和对照组,各60例。对照组均实施常规治疗;观察组在此基础上加用硫辛酸0.2 g,每日1次,在餐前30 min口服。两组患者疗程均为1个月。观察记录两组患者治疗前后的氧化应激指标、炎症指标,并记录其疗效。结果治疗后,观察组患者的总有效率显著高于对照组[85.0%(51/60)比65.0%(39/60)],差异有统计学意义(P <0.05)。治疗后,两组患者空腹血糖、餐后2 h血糖低于治疗前,差异有统计学意义(P<0.05)。治疗后,观察组血清晚期蛋白氧化产物、丙二醛低于对照组[(53±7) mmoL/L 比(58±9) mmoL/L,(4.7±0.5)μmoL/L 比(5.7±0.7)μmoL/L],超氧化物歧化酶高于对照组[(33±6) U/L比(30±6) U/L],差异有统计学意义( P<0.01)。治疗后,观察组血管细胞黏附分子1、成纤维细胞生长因子2、肿瘤坏死因子α、白细胞介素6均低于对照组[(810±205) g/L比(1004±266) g/L,(9.1±2.6) ng/L 比(12.4±2.7) ng/L,(14±4) pg/L比(18±5) pg/L,(13±4) pg/L比(18±5) pg/L],差异有统计学意义( P<0.01)。结论使用硫辛酸治疗早期糖尿病足可显著提高其疗效,并可有效改善氧化应激反应以及炎症的情况。%Objective To study the clinical effect of lipoic acid for treatment of early diabetic foot and the influence on oxidative stress and inflammation indexes .Methods Total of 120 patients with early diabe-tesfoot admitted to the Shengli Hospital of Chinese Petrochemistry Group Victory Petroleum Administrative Bureau from Jan.2014 to Mar.2015 were included in the study and divided into an observation group and a control group according to the random number table method,60 cases each group.The control group received conventional treatment;the observation group was added with thioctic acid, 0.2 g, once daily, orally 30 minutes before meal.The treatment courses for both groups was 1 month.The oxidized stress,inflammation indexes of the two groups were observed and recorded,and the clinical effect was recorded.Results After treatment,total effective rate of the observation group was significantly higher than the control group [85.0%(51/60) vs 65.0% (39/60)],the difference was statistically significant(P<0.05).After treatment,fast-ing blood glucose, postprandial 2 h blood sugar of two groups were lower than before treatment ,the difference was statistically significant(P <0.05).After treatment,the serum advanced oxidation protein products, malondialdehyde of the observation group were lower than the control group [ ( 53 ±7 ) mmoL/L vs ( 58 ± 9) mmoL/L,(4.7 ±0.5) μmoL/L vs (5.7 ±0.7) μmoL/L],superoxide dismutase was higher than the control group[(33 ±6) U/L vs (30 ±6) U/L],the difference was statistically significant(P<0.01).After treatment,vascular cell adhesion molecule-1,fibroblast growth factor-2,tumor necrosis factor α,interleukin-6 of the observation group were lower than the control group[(810 ±205) g/L vs (1004 ±266) g/L,(9.1 ± 2.6) ng/L vs (12.4 ±2.7) ng/L,(14 ±4) pg/L vs (18 ±5) pg/L,(13 ±4) pg/L vs (18 ±5) pg/L], the differences were statistically significant ( P<0.01 ) .Conclusion The lipoic acid treatment of early dia-betic foot can significantly improve the curative effect ,and effectively improve the condition of oxidative stress and inflammation .
