肾动脉阻力指数诊断早期肾脏损害的研究进展

         

摘要

Kidney damage refers to renal arterial lesions caused by renal ischemia,renal parenchymal fibrosis,which then develops into renal dysfunction,and eventually uremia.Hypertension,diabetes caused by target organ damage process is slow and hidden,often developing into proteinuria,serum creatinine after years,which is difficult to reverse once devel-oped into the end stage of renal disease,so early diagnosis and treatment of renal damage to improve the prognosis of patients and reduce mortality is important. As one of the most common complications of critically ill patients,acute kidney injury poses a threat to their lives.Clinical widely used serum creatinine diagnostic method is lagging behind,so finding a more sensitive and convenient diagnostic methods is to be resolved. Renal artery resistance index is obtained from Doppler ultrasound renal hemodynamics information,and as a new diagnostic method,it not only is of important clinical value in the prediction of kidney damage risk,severity assessment but also has unique advantages of no trauma,bedside application,eco-nomical,and repeatablility.%肾脏损害指由于肾脏小动脉病变引起肾脏缺血、肾实质纤维化,进而出现肾功能减退,最终导致尿毒症.高血压、糖尿病引起的肾脏靶器官损害过程缓慢且隐匿,常在多年后才出现蛋白尿、血肌酐升高等表现,一旦进入终末期肾病则难以逆转,因此肾脏损害的早期诊治对改善患者预后及降低病死率有重要意义.急性肾损伤是重症患者最常见的并发症之一,对其生命造成威胁.临床广泛应用的血肌酐诊断法相对滞后,因此寻找更为敏感便捷的诊断方法亟待解决.肾动脉阻力指数是由多普勒超声获得的肾脏血流动力学信息,作为新型诊断方法,不仅在肾脏损害的风险预测、严重程度评估等方面有重要临床价值,而且具有无创伤、床边化、廉价、可重复的独特优点.
