目的 分析结直肠癌患者奥沙利铂化疗不良反应发生率与临床病理特征的关系.方法 回顾性分析2013年4月至2014年4月上海交通大学医学院附属新华医院肛肠外科收治的137例应用奥沙利铂化疗的结直肠癌患者的临床资料,其中结肠癌72例、直肠癌65例;统计分析不良反应发生情况及与临床病理特征的关系.结果 137例结直肠癌术后化疗患者中, 112例患者发生各种类型的不良反应,其中不良反应发生率前三位的分别是周围神经毒性[62.8%(86/137)]、胃肠道反应[50.4%(69/137)]、静脉炎[46.0%(63/137)].年龄>65岁的不良反应发生率高于年龄≤65岁的患者,KPS评分≤80分的不良反应发生率高于KPS评分>80分的患者(P<0.05),肿瘤分期为A、B期的患者的不良反应发生率显著低于C、D期的患者(P<0.05).结论 奥沙利铂在结直肠癌患者的不良反应发生率较高,不良反应主要以周围神经毒性、胃肠道反应及白细胞、血小板减少为主,不同年龄、KPS评分、肿瘤分期患者的不良反应发生率不同.%Objective To analyze the relationship between the incidence of adverse reactions of oxaliplatin chemotherapy and clinicopathological features of patients with colorectal cancer.Methods A retrospective analysis on 137 patients of colorectal cancer treated with oxaliplatin in Xinhua Hospital Affiliated to Shanghai Jiaotong University School of Medicine from Apr.2013 to Apr.2014 was done,including 72 cases of colon cancer,65 cases of colorectal cancer;the incidence of adverse reactions and its relationship with clinical pathological characteristics was statistically recorded and analyzed.Results Among the 137 cases of postoperative chemotherapy of colorectal cancer,112 patients had various types of adverse reactions,the top three in terms of incidence were peripheral neurotoxicity[62.8%(86/137)],gastrointestinal tract reaction[50.4%(69/137)],and phlebitis[46.0%(63/137)].The incidence of adverse reactions of patients with age> 65 years old was higher than that of patients aged ≤65 years old,of patients with KPS score ≤80 scores was higher than that of patients with KPS score> 80 scores (P<0.05),the incidence of patients of stage A,B was significantly lower than that of the patients of stage C,D(P<0.05).Conclusion Oxaliplatin in colorectal cancer patients has a high rate of adverse reactions,mainly including peripheral neurotoxicity,gastrointestinal reactions and white blood cells,thrombocytopenia reduction,and the incidence varies in patients with different ages,KPS scores and tumor stages.
