高频超声在小儿急性肠套叠诊疗中的临床应用

摘要

目的 探讨高频超声在小儿急性肠套叠的诊断和治疗中的临床应用价值.方法 对48例临床疑诊急性肠套叠的小儿患者行高频超声检查,确诊患者在超声引导下采用0.9％ NaCl溶液灌肠复位治疗,对声像图特征及治疗结果进行分析.结果 在48例患者中,45例经高频超声准确诊断,符合率93.8％；漏误诊3例.小儿急性肠套叠的特征性超声图像改变是“同心圆”征或“靶环”征,“套筒”征或“假肾”征.其中43例患者在高频超声引导下进行了灌肠复位治疗,全部获得成功,成功率100％.结论 高频超声是早期诊断和灌肠复位治疗小儿急性肠套叠的有效方法,值得临床推广应用.%Objective To explore the clinical value of high frequency ultrasonography in diagnosis and treatment of children with acute intussusception. Methods 48 pediatric patients suspected acute intussusception weres checked using high frequency ultrasound. Under ultrasound guidance, patients diagnosed with acute intussusception were given enema treatment with 0. 9% NaCl solution. The sonographic features and treatment outcome were analyzed. Results Of 48 patients, 45 cases were detected by high frequency ultrasonography and the accuracy was 93. 8% (45/48). 3 cases were misdiagnosised. The typical imaging characteristics of acute intussusception in children were concentric circles sign, target ring sign, sleeve sign or false kidney sign. In the high-frequency ultrasound-guided, 43 patients were carried out with normal saline enema reduction with success rate of 100%. Conclusion The high-frequency ultrasound is an effective method of early diagnosis and treatment of acute intussusception in children.
