椎间孔椎间融合术治疗腰椎退行性疾病的临床疗效分析

摘要

目的 观察微创经椎间孔椎间融合术治疗腰椎退行性疾病的临床疗效.方法 选择2010年2月～2015年2月我院收治的腰椎退行性疾病患者96例,根据随机数字表法分为治疗组与对照组各48例,治疗组给予Quadrant微创经椎间孔椎间融合术,对照组给予开放经椎间孔椎间融合术.结果 所有患者都顺利完成手术,切口均Ⅰ期愈合,不过治疗组的切口长度、术中出血量、术后引流量与术后住院时间明显少于对照组(P＜0.05).两组术后3个月的脊髓神经功能都明显好于术前(P＜0.05),同时治疗组术后3个月的脊髓神经功能明显好于对照组(P＜0.05).治疗组与对照组术后3个月的Oswestry功能障碍评分为(13.45±4.18)分和(21.98±5.14)分,都明显低于术前的(32.98±4.11)分和(33.08±4.08)分(P＜0.05),同时组间差异也有统计学意义(P＜0.05).治疗组术后3个月内的褥疮、气胸、脑脊液漏、胃肠反应等不良反应发生情况明显少于对照组(P＜0.05).结论 微创经椎间孔椎间融合术治疗腰椎退行性疾病能减少对患者的创伤,促进脊髓神经功能与腰椎功能的恢复,安全性佳,有利于患者术后康复.%Objective To evaluate the clinical effects of minimally transforaminal interbody fusion for treating lumbar degenerative disease.Methods 96 patients with lumbar degenerative disease from February 2010 to February 2015 in our hospital were divided into the treatment group and the control group based on the random number table.The treatment group were received minimally invasive transforaminal Quadrant interbody fusion,and the control group were received open transforaminal lumbar interbody fusion.Results All patients were successfully operated and with Ⅰ incision healing.The incision length,blood loss,postoperative drainage and postoperative hospital stay in the treatment group were significantly less than that of the control group (P＜0.05).The postoperative 3-month neurological function were significantly better than the preoperative (P＜0.05),while the postoperative 3-month neurological function in the treatment group were significantly better than that of the control group (P＜0.05).The postoperative 3-month Oswestry Disability scores in the treatment group and control group were 13.45 ±4.18 and 21.98 ± 5.14,which were significantly lower than the preoperative (32.98±4.11,33.08±4.08) (P＜0.05).The postoperative 3-month bedsores,pneumothorax,cerebrospinal fluid leakage,gastrointestinal reactions in the treatment group were significantly less than that of the control group (P＜0.05).Conclusion Minimally invasive transforaminal lumbar interbody fusion for treating lumbar degenerative disease can promote the recovery of renal function and lumbar spinal cord,it has good security that is conducive to the rehabilitation of patients.
