研究术前超声检测直肠癌浸润深度的价值，为临床术前判断提供依据。选取2012年1月~2014年12月在我院进行超声检测的直肠癌患者49例，依据UICC 2002年公布的TNM分期标准对直肠癌进行分期，按照术前检测结果将患者分为UT1组5例、UT2组8例、UT3组19例、UT4组17例，其中进展期患者12例，早期患者37例。收集所有患者术后病理诊断、手术结果等资料对术前超声诊断的预判价值进行分析，计算检测准确度、灵敏度、特异度、阳性预测、阴性预测等。术前超声检测对直肠癌诊断总的准确率为79.59%，UT1、UT2、UT3和UT4的准确度分别为79.60%、80.03%、79.59%、79.63%。术前超声检测对直肠癌浸润深度具有很高的准确性，能为后续治疗方案的制定提供依据，值得临床推广。%To study the role of preoperative ultrasonic examination in determination of the invasion depth of rectal cancer and to provide the basis for preoperative clinical judgment. Total 49 patients with rectal cancers to receive ultrasonic examinations in our hospital from January 2012 to December 2014 were selected. They were graded on the basis of TNM grading standard issued by UICC in 2002 and divided into UT1 (n=5), UT2 group (n=8), UT3 (n= 19) and UT4 (n = 17) groups according to the preoperative examination results, including 12 ones in aggressive phase and 37 ones in early phase. The information, such as postoperative pathologic diagnosis and operative results, was collected to analyze the predictive role of preoperative ultrasonic diagnosis and calculate the accuracy, sensitivity, specificity, positive prediction and negative prediction etc. The overall accuracy of preoperative examination in detection of rectal cancer was 79.59%. The accuracies in UT1, UT2, UT3 and UT4 groups were 79.60%, 80.03%, 79.59% and 79.63%, respectively. Preoperative ultrasonic examination is highly accurate in prediction of the invasion depth of rectal cancer and can provide a basis for subsequent therapeutic schedule preparation, so it is worthy of a wider application.
