精神分裂症患者血清5-HT、MT、TSH水平的变化及临床意义

摘要

Objective To explore the changes and clinical significance of serum levels of serotonin (5-HT),Melatonine (MT) and thyroid stimulating hormone (TSH) of patients with schizophrenia.Methods A total of 160 patients with schizophrenia admitted into our hospital were taken as the research subjects and received 12-month anti-psychotic treatment.Respectively prior to and after six and 12 months' treatment,the PANSS scale was used to assess the severity of mental symptoms of the patients.Serum levels of 5-HT,MT and TSH were tested and the relationship between the total PANSS score and serum 5-HT,MT and TSH levels were analyzed.R esults After the six and 12 months' treatment,the positive symptom scale score,the negative symptom scale score,the general psychopathological scale score and the total PANSS score were significantly lower than those before the treatment (P ＜ 0.05),while the serum levels of 5-HT,MT and TSH increased significantly (P ＜ 0.05).In the comparison between the scores after 12 months' treatment and those after six months' treatment,the aforesaid indices also showed significant improvement (P ＜ 0.05);Pearson correlation analysis showed that the total PANSS was negatively correlated to the serum levels of 5-HT and TSH (P ＜ 0.05).Conclusion The serum levels of 5-HT,MT and TSH are negatively correlated to the total PANSS score of the patients with schizophrenia.Whether or not they can be used as objective indices for assessing patents' progress of disease and the effects of treatment requires further research and confirmation.%目的 探讨精神分裂症患者血清5-羟色胺(5-HT)、褪黑素(MT)、促甲状腺激素(TSH)水平的变化及意义.方法 将医院收治的160例精神分裂症患者作为研究对象,均给予积极的抗精神病治疗12个月.在治疗前及治疗6个月、12个月时,采用阳性和阴性症状量表(PANSS)评估患者的精神症状严重程度,检测血清5-HT、MT、TSH水平,并探讨PANSS总分与血清5-HT、MT、TSH的关系.结果 治疗6、12个月时,患者的阳性量表评分、阴性量表评分、一般精神病理量表评分和PANSS总分均较治疗前显著降低(P＜0.05),而血清5-HT、MT、TSH水平则显著上升(P＜0.05);治疗12个月与治疗6个月比较,上述指标也有显著改善(P＜ 0.05);Pearson相关分析显示,PANSS总分与血清5-HT、TSH呈负相关(P＜0.05).结论 血清5-HT、MT、TSH水平与精神分裂症患者PANSS总分呈负相关,可能作为评价患者病情程度和治疗效果的客观指标,有待进一步的研究证实.
