顺行穿刺腔内治疗在下肢动脉硬化闭塞症中的应用价值及疗效观察

摘要

目的：对应用顺行穿刺腔内治疗技术对患有下肢动脉硬化闭塞症的患者实施治疗的临床效果进行研究。方法：抽取94例患有下肢动脉硬化闭塞症的患者，随机分为对照组和治疗组，平均每组47例。对照组采用常规下肢动脉硬化手术方式实施治疗；治疗组采用顺行穿刺腔内治疗技术实施治疗，穿刺成功后将6 F血管鞘置入，并外接压力袋，加入肝素的平衡液灌注线持续高压滴注，通过造影进一步明确动脉狭窄的实际程度和具体部位，外周静脉给予5000 IU肝素，用超滑微导丝配合微导管插入病变血管狭窄段的远端，对狭窄段进行扩张处理，扩张时间应该持续60 s左右，尽量进行一次性扩张处理；对比两组患者的治疗效果。结果：治疗组患者下肢动脉硬化闭塞症治疗效果明显优于对照组；下肢功能复常时间和接受治疗总时间明显短于对照组；治疗前后下肢功能评分改善幅度明显大于对照组；围手术期不良反应率明显低于对照组。结论：应用顺行穿刺腔内治疗技术对患有下肢动脉硬化闭塞症的患者实施治疗的临床效果非常明显。%Objective:To study the application of antegrade puncture endovascular treatment techniques for the implementation of the clinical effect of treatment on patients with arteriosclerotic occlusive disease of lower extremity.Method:The total of 94 patients with lower extremity arteriosclerosis obliterans patients,randomly divided into treatment group and control group,each an average of 47 cases.The control group using conventional lower extremity arteriosclerosis operation mode to implement treatment;treatment group were treated with antegrade puncture endovascular treatment technology to implement treatment,after successful puncture 6F vascular sheath placement,and external pressure bag,equilibrium liquid filling line adding heparin continuous high pressure infusion,through contrast into the actual step clear artery stenosis and the specific site peripheral intravenous heparin,5000 IU,super slide wire combined with distal micro catheter insertion lesions of artery stenosis are expanded,the stenosis,the expansion time should last about 60 s,to a one-time expansion treatment;compared two groups of patients.Result:The treatment group in patients with lower limb arterial occlusive disease treatment effect is better than that of the control group;lower limb function recovery time and treatment time was shorter than the control group;lower limb function before and after treatment were improved significantly than those of the control group during operation period;adverse reaction rate significantly lower than the control group.Conclusion:The application of antegrade puncture transluminal therapy for patients with lower extremity arteriosclerosis obliterans of the implementation of the clinical treatment effect is very obvious.
