经皮肾镜碎石术治疗鹿角型肾结石清除率的影响因素

摘要

目的：分析经皮肾镜碎石术治疗鹿角型肾结石清除率的影响因素。方法：选取本院2015年2月-2016年5月接受治疗的鹿角型肾结石患者91例，分析全部的临床资料，通过单变量、多变量因素统计分析，剖析清除率的影响因素。结果：PNL术共计96次，清除率为54.17%（52/96）。经过单变量分析，结石形态、手术医师年资、手术通道与清除率均存在显著相关性（字2=3.857、3.024、4.214， P<0.05）。多变量因素结果表明，手术医师年资、结石形态、手术通道都属于清除率的独立危险因素（P<0.05）。结论：清除率的影响因素包括结石形态、手术医师年资、手术通道，为此应当注意这三个因素，安排经验丰富的医师为患者手术，强化培训，提高医师的手术操作技巧。%Objective:To analyze the influence factors of percutaneous nephrolithotomy for treatment of renal staghorn calculi clearance rate.Method:91 patients with antlers renal stones in our hospital from February 2015 to May 2016 were selected,the clinical data were analyzed,through the single variable factors,multivariate statistical analysis,the influence factors of clearance were analyzed.Result:PNL total of 96 times,the clearance rate was 54.17% (52/96).After univariate analysis results showed that the morphology of stone, the surgeon seniority, there were significant correlation between the channel and the clearance rate of operation (χ²=3.857, 3.024,4.214,P<0.05).Multivariate results show that surgeons seniority,calculus form,operation channel were independent risk factors for the removal rate (P<0.05).Conclusion:The factors affecting the clearance rate of including calculous form,operation medical qualification,operation channel,therefore should pay attention to these three factors,arrange experienced physicians for patients with surgery,intensive training,improving the operation skills.
