不同离心条件和抗凝剂对凝血功能检测结果的影响

         

摘要

目的:探讨不同离心条件和抗凝剂对凝血功能检测结果的影响。方法采用不同的离心速度和离心时间、两种不同的抗凝剂,用 STA‐R型全自动血凝仪分别测定其凝血酶原时间(PT )、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、抗凝血酶Ⅲ活性(ATⅢ)、纤维蛋白原(Fib)、D‐二聚体(D‐D)和纤维蛋白(原)降解产物(FDP)。结果与3000 r/min离心10 min组测定结果比较,除3000 r/min离心5 min组PT、TT、ATⅢ、Fib的差异无统计学意义外(P>0.05),其余各组的凝血功能检查结果差异均有统计学意义(P<0.05)。与枸橼酸钠抗凝血检测结果相比,乙二胺四乙酸二钾(EDTA‐K2)抗凝血使PT、APTT、TT延长,ATⅢ活性增高(P<0.05),而Fib、D‐D、FDP差异无统计学意义(P>0.05)。结论3000 r/min离心10 min为凝血功能检测的最佳离心条件,且测定Fib、D‐D、FDP时临床可用EDTA‐K2抗凝血替代,其余均必须使用枸橼酸钠抗凝血。%Objective To explore the influence of different centrifugal conditions and anticoagulants on the de‐tection results of blood coagulation function .Methods The prothrombin time(PT) ,activated partial thromboplastin time(APTT) ,thrombin time(TT) ,antithrombin Ⅲ (AT Ⅲ ) ,fibrinogen(Fib) ,D‐dimer(D‐D) and fibrin degradation products (FDP) by STA‐R type full automatic blood coagulation analyzer ,which were detected by adopting different centrifugal speeds and time and two different kinds of anticoagulant .Results Compared with the detection results in the 3 000 r/min 10 min centrifugation group ,the blood coagulation function test results had statistically significant differences among other centrifugal condition groups (P<0 .05) ,except PT ,TT ,ATⅢ and Fib in the 3 000 r/min 5 min centrifugation group had no statistical differences(P>0 .05) .Compared with the detection results of sodium cit‐rate anticoagulation ,the anticoagulation by using ethylenediamine tetraacetic acid dipotassium(EDTA‐K2 ) prolonged PT ,APTT ,TT and increased the ATⅢ activity(P<0 .05) ,while Fib ,D‐D and FDP had no statistical differences(P>0 .05) .Conclusion The best centrifugal condition for blood coagulation function detection is 3 000 r/min centrifu‐ging for 10 min ,and in the determination of Fib ,D‐D and FDP ,EDTA‐K2 anticoagulation can replace ,while the others must use sodium citrate anticoagulation .
