车针在老年磨牙残根残冠保存修复中的应用

摘要

Obj ective To investigate the clinical effects of using MANI dia-bur in metal post and core residual roots and crowns restoration in older people.Methods We selected 120 teeth in 90 old patients who had undergone residual root and crown restoration operations from February 2009 to August 2012 in our department.The 120 teeth were ran-domly divided into three groups,the experimental group metal post and core group using MANI burs in 30 patients(38 teeth),control group for casting metal post and core in 30 cases(43 teeth)and threaded titanium screw metal post re-pair in the 30 cases(39 teeth).We followed up the patients for 6 months to 3 years and compared the clinical effect of the restoration in three groups.Results The success rate was 96.7% for the first group,93.3% for the second and 93.3%for the third.There was no statistical significant difference in the success rate of restoration operations between three groups (P> 0.05).Conclusion MANI burs metal post and core clinical application has the same effect with cast metal post and core and titanium nail for residual roots and crowns restoration.It is an effective method to repair the residual roots and crowns,with the obvious advantages of shorter recovery,fewer complications,low cost,and easy operation.It can be applied widely to clinical teeth restoration surgery.%目的探讨金属桩核在修复老年人磨牙残根、残冠中的临床效果。方法选择于2009年2月-2012年8月在乌鲁木齐市口腔医院修复科行残根、残冠保存修复的老年患者90例120颗牙，随机分为3组：MANI车针金属桩核组30例38颗牙（MANI车针组）、铸造金属桩核修复30例43颗牙（铸造金属桩核组）和螺纹钛钉金属桩核修复30例39颗牙（螺纹钛钉组）。随访6个月～3 a，比较3组桩核修复后的临床应用效果。结果经过6个月～3 a的随访观察，MANI车针组出现1例冠脱落，成功率为96．7％。铸造金属桩核组出现1例冠脱落，1例桩核冠脱落，成功率为93．3％。螺纹钛钉组出现2例冠脱落，成功率为93．3％。MANI车针桩核修复的成功率略高于铸造金属桩核和螺纹钛钉桩核修复的成功率，三者比较差异无统计学意义，3组之间临床应用效果两两比较，差异无统计学意义（P ＞0．05）。结论 MANI车针金属桩核应用于临床，与铸造金属桩核和螺纹钛钉有着相同的应用效果，是残根残冠保存修复的有效方法，能有效提高患者修复后的即时疗效，减轻修复后长、短期内各项并发症的发生，而且成本低廉、操作简便，可以在口腔临床修复中推广使用。
