目的 探讨橡皮障在儿童口腔根管治疗中的应用价值.方法 选取 2016 年 12 月-2017 年 5 月于乌鲁木齐市第一人民医院(儿童医院)口腔科行口腔根管治疗的患儿为研究对象.本研究共纳入 43 例患儿,男 26例,女 17 例,年龄 8～16 岁,平均 10.3 岁,共47 颗年轻恒后牙.按治疗顺序将其分为观察组(22 例,24 颗患牙)和对照组(21 例,23 颗患牙).实验组在操作中放置橡皮障,对照组在根管治疗中未放置橡皮障,使用棉卷和强吸隔湿.其他所有治疗步骤 2 组均按常规进行.患者治疗后,医护人员对患者进行 Nege评分.7 d后患者随访时检查X线,对患者进行疗效的评价,对填充效果进行评价.结果 观察组患者每颗患牙的治疗平均操作时间为(17.3± 2.3)min,对照组平均每颗患牙操作时间为(23.4±3.7)min,观察组患者的平均治疗操作时间明显优于对照组,经比较差异有统计学意义(t＝-7.186,P ＝0.000).各级 Nege结果所占比例差异有统计学意义(χ2＝ 10.651,P ＝0.014).发生急性炎症反应情况比较结果显示,观察组发生急性炎症反应患儿(即 Nege评级为 3～4 级)比例明显高于对照组,经比较差异有统计学意义(χ2＝10.000,P＝0.002).对照组恰填17 颗,欠填3 颗,超填2 颗.观察组恰填 15 颗,欠填 2 颗,超填 1 颗.2 组填充效果比较差异无统计学意义(χ2＝0.659,P ＝0.720).结论 利用橡皮障技术辅助口腔治疗患儿根管,能明显减少患者的术后炎症反应,与不放置橡皮障的根管治疗的填充效果相当,该技术在辅助治疗口腔方面有一定的优势.%Objective To investigate the value of rubber dam in the treatment of children′s oral root canal.Methods From December 2016 to May 2017,children undergoing oral root canal therapy in the First Peo-ple′s Hospital of Urumqi(Children's Hospital)were selected as the research objects.A total of 43 chil-dren,26 males and 17 females,aged from 8 to 16 years and an average of 10.3 years old,were enrolled in this study.A total of 46 young permanent posterior teeth were included.They were divided into observa-tion group(n＝22)and control group(n＝21)by treatment order.The experimental group placed a rub-ber dam in operation.The control group did not place a rubber dam in the root canal treatment,the use of cotton swab and strong suction from the wet.All other treatment steps were routinely performed in both groups.After the patient was treated,the health care provider performed a Nege score on the patient.Af-ter 7 days,the patients were examined by X-ray at follow-up.The curative effect was evaluated and the e-valuation of filling effect was evaluated.Results The average operation time of each tooth in treatment group was(17.3±2.3)min and that in control group was(23.4±3.7)min.The average treatment time in observation group was significantly higher than that of control group,the difference was statistically sig-nificant(t＝－7.186,P＝0.000).Nege results at all levels of the proportion of differences was statistically significant(χ2＝10.651,P ＝0.014).The incidence of acute inflammatory reactions compared with the re-sults showed that the incidence of acute inflammation in the observation group(ie Nege grade 3 to 4)was significantly higher than the control group,the difference was statistically significant(χ2＝10.000,P ＝0.002).Control group just fill in 17 teeth,3 teeth due to fill,over fill in 2 teeth.The observation group just filled in 1 7 teeth,under-fill in 2 teeth,over-filling in 1 tooth.There was no significant difference in the filling effect between the two groups(χ2＝0.659,P ＝0.720),but the filling effect was similar in the two groups.Conclusion The use of rubber dam technology to assist oral treatment of children with root canal can significantly reduce the postoperative response of patients,suggesting that the technology in the adju-vant treatment of oral has some advantages.
