Klotho蛋白与心力衰竭患者肾脏损害的相关性

摘要

目的 研究klotho蛋白与心力衰竭患者肾脏损害的相关性.方法 收集我院2009年3月至2010年12月间住院的115例心力衰竭患者血样本,采用ELISA方法检测klotho蛋白,分析不同心力衰竭的程度、不同血肌酐(Cr)水平与klotho蛋白之间的关系.结果 心力衰竭患者NYHA Ⅰ～Ⅱ级、Ⅲ级及Ⅳ级血清klotho蛋白吸光度分别为0.45±0.12、0.35±0.13及0.24±0.12,组间比较差异具有统计学意义(P＜0.05)；心力衰竭A组、心肾综合征B组及心肾综合征C组患者血清klotho蛋白吸光度分别为0.43±0.12、0.33±0.13及0.22±0.13,组间比较差异具有统计学意义(P＜0.05).Klotho蛋白与Cr水平呈负相关(r=-0.517,P＜0.01),而与左室射血分数(LVEF)呈正相关(r=0.64,P＜0.01).结论 心力衰竭的程度及其合并肾脏损伤程度与低水平klotho蛋白有关,klotho蛋白可以作为判断心力衰竭病情严重程度的指标之一.%Objective To study the relationship between klotho protein and heart failure associated with kidney damage. Methods The blood samples of 115 heart failure patients hospitalized between March 2009 and December 2010 were collected; klotho protein was detected by ELISA. The relationship of different severity of heart failure and different serum creatinine (Cr) level with klotho protein was analyzed. Results Klotho protein absorbance level of patients with heart failure class NYHA I - H , IE and IV grade was 0. 45±0.12, 0.35±0.13 and 0.24±0.12, respectively, with significant differences among the groups (P＜0.05); klotho protein absorbance of patients with heart failure A group, cardiorenal syndrome B group and group C was 0.43±0.12, 0.33±0.13 and 0.22 ± 0.13, respectively, with significant differences among the groups (P＜0.05). Klotho protein was negatively correlated with Cr level (r = - 0.517, P＜0.01), but positively correlated with LVEF (r = 0.64, P＜0.01). Conclusion The more severe heart failure and kidney damage are, the lower klotho protein level is; therefore, klotho protein can be used as one of the indicators to judge the severity of heart failure.
