人工听骨与自体听骨在听骨链重建术后的疗效观察

摘要

Objective To observe the curative efficacy of artificial auditory ossicles and autogenous ossicles in ossicular chain recon struction.Methods The clinical data of 80 patients(80 ears) with ossicular reconstruction in our hospital from April 2015 to April 2017 were statistically analyzed.The patients were divided into artificial otosteon group (n =40,40 ears) and autologous ossicular group (n =40,40 ears) according to the treatment methods.The pure tone audiometry hearing improvement and incidences of postoperative complications and recurrence,operation time,operation costs between the two groups were statistically analyzed.Results The postoperative complications and recurrence rate were significantly lower in autologous ossicular group than in artificial otosteon group (2.5％ vs 10.0％,0％ vs 10.0％,P ＜0.05).The operation time was significantly longer in autologous ossicular group than in artificial otosteon group(P ＜ 0.05).The operation cost was significantly lower in autologous ossicular group than in artificial otosteon group (P ＜ 0.05).But the percentage of audiometric hearing increased by 30,25,20,15,10 dB between the two groups showed not significant difference(P ＞ 0.05).Conclusion The curative efficacy of artificial auditory ossicles and autogenous ossicular ossicles for ossicular chain reconstruction is similar.The autogenous ossicular ossicles has longer operation time than the former,but lower postoperative complication rate,recurrence rate and operation cost.%目的 观察人工听骨与自体听骨在听骨链重建术后的疗效. 方法 对我院2015-04～2017-04收治的80例(80耳)行听骨链重建术的患者的临床资料进行统计分析,依据治疗方法将这些患者分为人工听骨组(40例40耳)和自体听骨组(40例40耳),对两组患者的纯音测听听力提高情况、术后并发症发生情况及复发情况、手术时间、手术费用进行统计分析.结果 自体听骨组患者的术后并发症发生率为2.5％(1/40),复发率为0％ (0/40),均显著低于人工听骨组[10.0％ (4/40),10.0％ (4/40)] (P ＜0.05),手术时间显著长于人工听骨组(P＜0.05),手术费用显著低于人工听骨组(P＜0.05),但两组患者的纯音测听听力提高30,25,20,15,10 dB、未提高比例之间的差异均不显著(P＞0.05). 结论 人工听骨与自体听骨在听骨链重建术后的疗效相当,后者较前者具有较长的手术时间,但具有较低的术后并发症发生率、复发率及手术费用.
