应用彩色多普勒联合多巴酚丁胺负荷诊断无症状心肌缺血患者护理配合

摘要

目的：分析应用彩色多普勒联合多巴酚丁胺负荷诊断无症状心肌缺血患者护理效果。方法选取2013年2月至2015年2月在我院自愿接受检查的体检者为研究对象，其中无症状心肌缺血患者82例为观察组、另选择同期检查的正常健康人82例为对照组。两组均采用美国GE公司的GE730型彩色超声诊断仪联合多巴酚丁胺诊断。对比两组负荷下的Ve值（峰值舒张早期充盈速度）、Va值（峰值舒张晚期充盈速度）及RIVRT（ e峰减速时间、等容舒张时间） LVEF值（左室射血分数）。观察组进行护理配合，对比护理前后的满意度及心理状况。结果观察组RIVRT值为（74．25±16．63） ms，LVEF值为（57．03±4．54）‰，与对照组比较有明显差别（均P＜0．05）。观察组Ve值及Ve／Va值均明显低于对照组（均P＜0．05）。护理后非常满意患者占比54．88％（45／82），总满意率为8．54％（7／82），均显著高于护理后（均P＜0．05）。护理后的HAMA评分为（12．1±9．7）分，HAMD评分为（11．4±6．7）分，均明显低于护理前（均P＜0．05）。结论应用彩色多普勒联合多巴酚丁胺负荷可明确评价无症状心肌缺血患者的左室舒张功能。在护理的配合下不仅可提高患者对医务人员的满意度，还可消除患者的焦虑感、紧张感，大大提高患者的治疗依从性。%Objective To analyze the nursing effect during the application of color Doppler combined with dobutamine stress echocardiography in diagnosing myocardial ischemia symptoms .Methods From February 2013 to February 2015, patients who accepted voluntary examination in our hospital as the research object , the 82 cases of patients with silent myocardial ischemia in the observation group , another 82 cases of healthy people over the same period examination as control group .The diagnosis was done by using dobutamine combined with type GE730 color ultrasonic diagnostic apparatus applied by America GE Company .Compared the two groups of load value of Ve ( peak early diastolic velocity ( Va ) , peak late diastolic filling velocity ( E ) and RIVRT peak deceleration time, isovolumetric relaxation time (LVEF) values of left ventricular ejection fraction ).In addition, the nursing satisfaction and mental health of observation group were compared before and after care .Results In the observation group, RIVRT value was (74.25 ±16.63) ms, LVEF was (57.03 ±4.54)%, there was significant difference when compared with the control group (all P <0.05).After loading, Ve value and Ve/Va value of the observation group were significantly lower than the control group (all P<0.05).The patients those were very satisfied with the nursing accounted for 54.88% (45/82), the total satisfactory rate was 8.54% (7/82), which were significantly higher than that before nursing (all P<0.05).HAMA score after nursing was (12.1 ±9.7) points, HAMD score was (11.4 ±6.7), which were significantly lower than that before treatment . The differences were statistically significant ( all P <0.05).Conclusions The application of color Doppler combined with dobutamine can clearly assess left ventricular diastolic function of patients with myocardial ischemia.In the nursing cooperation , not only can improve the patients'satisfaction of the medical staff , but also can eliminate patient anxiety , tension, improve the treatment compliance of patients .
