喙肩韧带部分移位修复重建喙锁韧带加锁骨钩钢板治疗急性肩锁关节 Tossy Ⅲ型脱位的疗效观察

Objective To investigate the operation methods and clinical value of combining coracoacromial ligament transposition partly to repair and reconstruct coracoclavicular ligament, with Clavicular Hook Plate for treatment of acute Tossy III acromioclavicular joint dislocation.Methods From September 2011 to October 2014, the total 12 patients with acute Tossy III acromioclavicular joint dislocation were treated by combining Coracoacromial ligament transposition partly to repair and reconstruct Coracoclavicular ligament, with Clavicular Hook Plate.Results The 12 patients were followed-up from 3 to 36 months, mean 16 months, all of them in the shoulder shape, motion range of the shoulder joint and X-ray examination of the acromioclavicular joint show that have normal relationship.Evaluated by the Karlsson standard: excellent 11 cases, good 1case. Conclusions For treatment of acute Tossy III acromioclavicular joint dislocation , the method of combining coracoacromial ligament transposition partly to repair and reconstruct coracoclavicular ligament , with Clavicular Hook Plate, can allow early functional exercise, functional recovery of the shoulder well.The results show that it is an effective method for treatment of acute Tossy III acromioclavicular joint dislocation.%目的：探讨喙肩韧带部分移位修复重建喙锁韧带加锁骨钩钢板治疗急性肩锁关节Tossy Ⅲ型脱位的手术方法及临床疗效。方法自2011年9月至2014年10月对12例急性肩锁关节Tossy Ⅲ型脱位行喙肩韧带部分移位修复重建喙锁韧带加锁骨钩钢板进行治疗。结果本组病例均获随访，随访时间3～36个月，平均随访16个月，患者在肩关节外形，肩关节活动度，X线片复查肩锁关节解剖关系正常，参照Karlsson标准进行肩关节功能评定：优11例，良1例。结论应用喙肩韧带部分移位修复重建喙锁韧带加锁骨钩钢板治疗急性肩锁关节Tossy Ⅲ型脱位，可早期功能锻炼，肩关节功能恢复好，是一种治疗急性TossyⅢ型肩锁关节脱位的有效方法。
