医院参与社区家庭管理模式对老年病患者生活质量的影响

摘要

Objective:To explore the impact of the model of hospital participation in family management in community on quality of life of patients with senile diseases. Methods:100 elderly patients who were discharged from hospital and registered in community were divided into the control group and the study group(50 cases in each group)according to the order of discharge and registration number in communi-ty. The conventional family management model was used in the control group and the model of hospital participation in family management in community was adopted in the study group. SF-36 scale was used to evaluate the quality of life of the elderly patients in the two groups before the intervention and after the intervention for 6 months respectively,and then the change in each dimension of quality of life of the patients was compared between the two groups. Results:After the intervention for 6 months,the scores of each dimension were higher in the study group than the control group(P﹤0. 01,P﹤0. 05). Conclusion:The model of hospital participation in family management in commu-nity can promote the rehabilitation and improve the quality of life of elderly patients with senile diseases.%目的：探讨医院参与社区家庭管理模式对老年病患者生活质量的影响。方法：将老年康复科出院及社区登记在册的老年病患者100例按出院及登记的先后顺序编号，奇数50例为对照组，采用常规的家庭管理照护模式；偶数50例为研究组，采取医院参与社区家庭管理模式，根据老年人综合评估（ CGA）结果制订医院参与社区家庭联合管理模式干预措施。于干预前和干预6个月后使用SF-36量表对两组患者进行生活质量评定，比较生活质量各维度的变化。结果：干预6个月后，研究组各项维度评分均高于对照组（ P﹤0.05，P﹤0.01）。结论：医院参与社区家庭管理模式可促进老年病患者的康复，提高老年病患者生活质量。
