首发抑郁症患者外周血microRNA-124的变化及临床意义

摘要

目的 观察血清微小RNA-124(miR-124)在首发抑郁症患者与健康人群之间的表达差异及治疗前后的水平变化,探讨患者血清miR-214的表达变化及临床意义.方法 选取首发抑郁症患者(病例组)100例和健康对照(对照组)30例,治疗前测定所有参与者外周血清 miR-214表达水平及病例组汉密尔顿抑郁量表(HAMD)评分,分别在治疗后第1、2、4、8周末再次测定病例组miR-214表达水平,研究终点再次评定病例组HAMD评分;根据HAMD减分率将患者分成4组,A组(减分率≥75%)、B组(50%≤减分率＜75%)、C组(25%≤减分率＜50%)、D组(减分率＜25%).比较病例组与健康人群外周血清miR-124表达水平的差异及病例组在治疗前后 miR-124水平的变化,分析治疗效果不同的4组miR-124表达水平的差异及其临床意义.结果 病例组miR-124表达水平高于对照组(P＜0.05),治疗后第4、8周末miR-124表达水平较治疗前下降(P＜0.05).治疗后四组比较差异有统计学意义(P＜0.05),进一步两两比较发现,A组miR-124表达水平低于B组、C组、D组(P＜0.05),D组miR-124表达水平高于B组和C组(P＜0.05).结论 抑郁症患者存在着miR-124表达水平异常升高,miR-124表达水平与抑郁症的诊断、治疗效果关系密切,可作为诊断和评估患者预后的生物学指标.%Objective To observe the differential expression and the level changes before and after treatment of serum microRNA-124 (miR-124) in patients with first-episode depression and healthy people , and to explore the changes of serum miR-214 expression and its clinical significance.Methods A total of 100 patients with first-episode depression ( case group) and 30 healthy controls (control group) were selected.The serum miR-214 expression levels and the Hamilton Depression Rating Scale (HAMD) scores of all patients were determined before treatment.After the first, second, fourth and eighth weekends , the expression of miR-214 in the case group was measured again after treatments at the end of 1st, 2nd, 4th, and 8th week.And at the end of the study assessed the HAMD score of the case group again ; the patients were divided into four groups according to the HAMD subtraction rate, Group A ( decrease rate ≥75%), Group B (50% ≤ reduction rate ＜75%), Group C (25% ≤reduction rate ＜50%), and Group D (decrease rate ＜25%).To compare the difference of serum miR-124 expression levels between the case group and healthy population and the changes of miR-124 levels before and after treatment in the case group , it analyzed the differences in the expression levels of miR-124 and the clinical significance of the four groups with different treatment effects.Results The expression level of miR-124 in the case group was higher than that in the control group (P＜0.05).The expression level of miR-124 was decreased at the 4th and 8th week after treatment compared with before treatment (P＜0.05). After treatment, the difference between the four groups was statistically significant (P＜0.05).Further comparisons between the two groups showed that the expression level of miR-124 in Group A was lower than that in Group B, C, and D (P＜0.05), and the expression level of miR-124 in group D was higher than that of Group B and C (P＜0.05).Conclusion There is an abnormal increase in the expression level of miR-124 in patients with depression.The expression level of miR-124 is closely related to the diagnosis and treatment of depression.It can be used as a biological indicator for the diagnosis and evaluation of prognosis.
